Remote Virtual Chronic Care RN

Support members with chronic conditions — RN-led chronic care planning, chronic condition education, and ongoing condition support from home.

$32–$54 / hour 100% Remote Full-Time / Part-Time Multiple U.S. States

Job Overview

Virelia Health connects registered nurses with opportunities across virtual chronic care management, chronic condition nursing, and digital chronic care programs.

We are seeking qualified Virtual Chronic Care RNs for remote opportunities supporting virtual chronic care programs, health systems, health plans, accountable care organizations (ACOs), and digital chronic care organizations.

A Remote Virtual Chronic Care RN supports members with chronic conditions — RN-led chronic care planning, chronic condition education, and ongoing condition support. Working within their state licensure and clinical scope of practice, the virtual chronic care RN supports members with conditions like diabetes, hypertension, COPD, heart failure, and asthma through virtual assessment, education, self-management support, and care plan reinforcement.

This position is well-suited to a registered nurse with chronic care management, ambulatory care, disease management, or virtual care nursing experience who wants to practice in a virtual setting — providing meaningful clinical chronic care work with schedule flexibility and without the demands of a facility-based environment.

The specific virtual chronic care program, condition focus, clinical model, and scope of responsibilities will depend on the employer and assignment.

This opportunity may be suitable for professionals searching for remote virtual chronic care RN jobs, work-from-home chronic care RN jobs, remote chronic care nursing jobs, virtual chronic care nurse, or remote disease management RN roles.

Scheduled Virtual Chronic Care with Predictable Hours

Virtual chronic care nursing runs on scheduled member outreach and condition support blocks, giving your clinical day predictability and structure. Programs typically offer daytime, evening, or weekend coverage to accommodate member availability. Many programs offer flexible 8- or 10-hour shifts, and some RNs work 4-day compressed weeks or part-time schedules. There are no facility floor assignments and no physical demands. We offer both full-time and part-time positions. Discuss your preferred schedule during your interview.

What You'll Do

As a Remote Virtual Chronic Care RN, you may support members across the full virtual chronic care lifecycle.

  • Conduct virtual chronic care assessments — clinical, functional, and self-management needs.
  • Develop individualized virtual chronic care plans in collaboration with members, caregivers, and the care team.
  • Provide condition-specific education — diabetes, hypertension, COPD, heart failure, and asthma.
  • Support members in monitoring their condition — blood sugar, blood pressure, symptoms, and weight.
  • Support medication understanding and adherence.
  • Reinforce provider care plans and clinical instructions.
  • Support members in setting and achieving condition management goals.
  • Support members in recognizing warning signs and when to seek care.
  • Support lifestyle modification — nutrition, activity, and smoking cessation.
  • Coordinate care with providers, specialists, and community resources.
  • Document each member contact accurately in the chronic care platform.
  • Identify members with rising risk or unmet needs and escalate to the appropriate clinical team.
  • Support members in crisis — following crisis escalation protocols.
  • Participate in virtual chronic care quality improvement, clinical review, and education.
  • Maintain current knowledge of chronic care guidelines, virtual care best practices, and licensure requirements.
  • Protect member privacy and maintain HIPAA compliance across all virtual chronic care interactions.

Virtual Chronic Care Nursing Focus

Virtual chronic care nursing is a rapidly growing area of clinical practice — driven by increased demand for chronic condition support, payer investment in chronic care management, and health system investment in digital care delivery. Virtual chronic care RNs extend clinical reach, improve chronic condition outcomes, and support members with ongoing condition management.

Depending on the program, virtual chronic care nursing may focus on:

Diabetes support
Hypertension support
COPD support
Heart failure support
Asthma support
Self-management education

The virtual chronic care RN's role is to provide virtual chronic care nursing within their licensure and scope of practice — including assessment, condition-specific education, self-management support, care plan reinforcement, documentation, and care coordination. Clinical decisions, medication changes, and diagnostic decisions belong to the treating providers; the virtual chronic care RN supports the member's chronic care and coordinates with the care team.

This structure keeps the role focused on chronic care assessment, condition-specific education, self-management support, documentation, and appropriate clinical escalation.

Condition Education & Self-Management Support

Effective virtual chronic care nursing depends on strong condition education and self-management support — the RN's clinical judgment and patient education skills determine how to help each member achieve their condition management goals.

Depending on the program and clinical focus, you may support members by:

  • Providing condition-specific education — diabetes, hypertension, COPD, heart failure, and asthma.
  • Supporting members in monitoring their condition — blood sugar, blood pressure, symptoms, and weight.
  • Supporting medication understanding and adherence.
  • Reinforcing provider care plans and clinical instructions.
  • Supporting members in setting and achieving condition management goals.
  • Supporting members in recognizing warning signs and when to seek care.
  • Supporting lifestyle modification — nutrition, activity, and smoking cessation.
  • Coordinating care with providers, specialists, and community resources.
  • Documenting each member contact accurately and consistently.
  • Escalating concerning clinical findings to the appropriate clinical team.
  • Following crisis escalation protocols when members report urgent clinical symptoms.

Clinical judgment, patient education skill, and consistency are essential — virtual chronic care nursing requires a strong patient-relationship orientation and the ability to help members make sustainable condition management changes.

Virtual Chronic Care Workflow

A Remote Virtual Chronic Care RN may follow a structured workflow across each virtual chronic care encounter.

Depending on the employer's protocols, you may:

Review Member Panel Virtual Outreach Assess Condition Educate & Support Reinforce Care Plan Document Encounter Escalate If Needed

Clinical Escalation & Member Safety

If a member reports acute clinical symptoms, urgent safety concerns, or other urgent needs during any virtual interaction, the RN must follow the employer's clinical escalation protocol immediately — including directing the member to call 911, contact their provider, or seek emergency care as appropriate. Virtual care does not replace emergency care.

Required Qualifications

  • Active RN license in good standing in the state(s) required by the employer.
  • Previous clinical experience in chronic care management, ambulatory care, disease management, or virtual care nursing.
  • Strong chronic condition management, patient education, and self-management support skills.
  • Comfortable with chronic care management platforms, telehealth platforms, and EHR systems.
  • Strong documentation and time-management skills.
  • Ability to recognize situations requiring clinical escalation.
  • Ability to work independently in a remote environment.
  • Ability to maintain confidentiality and HIPAA compliance.
  • Reliable internet suitable for real-time video and a private workspace suitable for confidential member conversations.

A Typical Day

A typical day may begin by reviewing your virtual chronic care panel — checking member names, recent condition metrics, care plan goals, and any condition outreach that is due.

You'll conduct virtual member outreach by phone or video — assessing clinical, functional, and self-management needs, and providing condition-specific education and self-management support.

Between member encounters, you'll reinforce provider care plans, coordinate with providers and specialists, and document each member contact accurately.

Throughout the day, you'll support members in monitoring their condition, support medication understanding and adherence, escalate concerning clinical findings to the appropriate clinical team, and follow up with members who need extra support.

The role combines virtual chronic care assessment, condition-specific education, self-management support, care plan reinforcement, documentation, and appropriate clinical escalation.

Unlike broader virtual primary care, virtual chronic care nursing focuses specifically on chronic condition management — you're the RN who helps members build the skills and confidence to manage their condition day to day.
