Remote Telehealth Care Coordinator RN

Support virtual care coordination — RN-led virtual care coordination, interdisciplinary care team coordination, and virtual care planning from home.

$32–$55 / hour 100% Remote Full-Time / Part-Time Multiple U.S. States

Job Overview

Virelia Health connects registered nurses with opportunities across virtual care coordination, telehealth care management, and digital care programs.

We are seeking qualified Telehealth Care Coordinator RNs for remote opportunities supporting virtual care companies, health systems, health plans, accountable care organizations (ACOs), and digital care organizations.

A Remote Telehealth Care Coordinator RN supports virtual care coordination — RN-led virtual care coordination, interdisciplinary care team coordination, and virtual care planning. Working within their state licensure and clinical scope of practice, the telehealth care coordinator RN supports members through virtual care planning, interdisciplinary team coordination, care navigation, and care plan reinforcement.

This position is well-suited to a registered nurse with care coordination, virtual care, telehealth, chronic care management, or ambulatory care nursing experience who wants to practice in a virtual setting — providing meaningful clinical coordination work with schedule flexibility and without the demands of a facility-based environment.

The specific telehealth care coordination program, member population, clinical model, and scope of responsibilities will depend on the employer and assignment.

This opportunity may be suitable for professionals searching for remote telehealth care coordinator RN jobs, work-from-home care coordination RN jobs, remote RN care coordinator jobs, virtual care coordinator RN, or remote telehealth care management RN roles.

Scheduled Virtual Care Coordination with Predictable Hours

Virtual care coordination runs on scheduled outreach and coordination blocks, giving your clinical day predictability and structure. Programs typically offer daytime, evening, or weekend coverage to accommodate member availability. Many programs offer flexible 8- or 10-hour shifts, and some RNs work 4-day compressed weeks or part-time schedules. There are no facility floor assignments and no physical demands. We offer both full-time and part-time positions. Discuss your preferred schedule during your interview.

What You'll Do

As a Remote Telehealth Care Coordinator RN, you may support members across the full virtual care coordination lifecycle.

  • Conduct virtual member assessments — clinical, functional, and psychosocial needs.
  • Develop individualized virtual care plans in collaboration with members, caregivers, and the interdisciplinary team.
  • Coordinate care across providers, specialists, and community resources through virtual channels.
  • Support members with chronic conditions — education, self-management support, and goal setting.
  • Support care transitions — post-discharge, post-ED, and specialist referrals.
  • Facilitate interdisciplinary care team coordination — virtual case conferences and care team huddles.
  • Coordinate with virtual care teams — providers, care managers, and behavioral health.
  • Reinforce provider care plans and clinical instructions.
  • Document all member contacts accurately in the care management platform.
  • Identify members with rising risk or unmet needs and escalate to the appropriate clinical team.
  • Support members in crisis — following crisis escalation protocols.
  • Coordinate with health plans, ACOs, and community resources.
  • Participate in virtual care quality improvement, clinical review, and education.
  • Maintain current knowledge of care coordination best practices, virtual care models, and licensure requirements.
  • Protect member privacy and maintain HIPAA compliance across all virtual care coordination interactions.

Telehealth Care Coordination Focus

Virtual care coordination is a rapidly growing area of clinical practice — driven by increased demand for virtual care, payer investment in care coordination, and health system investment in digital care delivery. Telehealth care coordinator RNs extend clinical reach, improve care coordination, and support members through virtual channels.

Depending on the program, telehealth care coordination may focus on:

Virtual member assessment
Virtual care planning
Interdisciplinary coordination
Chronic care support
Care transitions
Member education

The telehealth care coordinator RN's role is to provide virtual care coordination within their licensure and scope of practice — including assessment, care planning, interdisciplinary coordination, care plan reinforcement, documentation, and care navigation. Clinical decisions, medication changes, and diagnostic decisions belong to the treating providers; the telehealth care coordinator RN supports the member's virtual care and coordinates with the care team.

This structure keeps the role focused on virtual assessment, care planning, interdisciplinary coordination, documentation, and appropriate clinical escalation.

Interdisciplinary Coordination & Virtual Care Planning

Effective virtual care coordination depends on strong interdisciplinary coordination and virtual care planning — the RN's clinical judgment and coordination skills determine how to help each member achieve their health goals.

Depending on the program and clinical focus, you may support members by:

  • Conducting virtual member assessments — clinical, functional, and psychosocial needs.
  • Developing individualized virtual care plans with members, caregivers, and the interdisciplinary team.
  • Facilitating interdisciplinary care team coordination — virtual case conferences and care team huddles.
  • Coordinating care across providers, specialists, and community resources through virtual channels.
  • Supporting members with chronic conditions — education, self-management support, and goal setting.
  • Supporting care transitions — post-discharge, post-ED, and specialist referrals.
  • Reinforcing provider care plans and clinical instructions.
  • Documenting each member contact accurately and consistently.
  • Escalating concerning clinical findings to the appropriate clinical team.
  • Following crisis escalation protocols when members report urgent clinical symptoms.

Clinical judgment, coordination skills, and clear communication are essential — virtual care coordination requires strong interdisciplinary collaboration and consistent member engagement across virtual channels.

Virtual Care Coordination Workflow

A Remote Telehealth Care Coordinator RN may follow a structured workflow across each virtual care coordination encounter.

Depending on the employer's protocols, you may:

Review Member Panel Virtual Outreach Assess Needs Develop Care Plan Coordinate Care Team Document Encounter Escalate If Needed

Clinical Escalation & Member Safety

If a member reports acute clinical symptoms, urgent safety concerns, or other urgent needs during any virtual interaction, the RN must follow the employer's clinical escalation protocol immediately — including directing the member to call 911, contact their provider, or seek emergency care as appropriate. Virtual care does not replace emergency care.

Required Qualifications

  • Active RN license in good standing in the state(s) required by the employer.
  • Previous clinical experience in care coordination, virtual care, telehealth, chronic care management, or ambulatory care nursing.
  • Strong care coordination, virtual care, and interdisciplinary coordination skills.
  • Comfortable with care management platforms, telehealth platforms, and EHR systems.
  • Strong documentation and time-management skills.
  • Ability to recognize situations requiring clinical escalation.
  • Ability to work independently in a remote environment.
  • Ability to maintain confidentiality and HIPAA compliance.
  • Reliable internet suitable for real-time video and a private workspace suitable for confidential member conversations.

A Typical Day

A typical day may begin by reviewing your virtual care coordination panel — checking member names, recent clinical notes, upcoming care coordination needs, and any care plan goals that need follow-up.

You'll conduct virtual member outreach by phone or video — assessing clinical, functional, and psychosocial needs, and coordinating with members and their care teams on care plan development and reinforcement.

Between member encounters, you'll facilitate interdisciplinary care team coordination — virtual case conferences and care team huddles — coordinate care across providers and specialists, and document each member contact accurately.

Throughout the day, you'll support members with chronic conditions, coordinate care transitions, escalate concerning clinical findings to the appropriate clinical team, and follow up with members who need extra support.

The role combines virtual member assessment, virtual care planning, interdisciplinary care team coordination, documentation, and appropriate clinical escalation.

Unlike direct virtual care nursing roles, virtual care coordination focuses on the connective tissue of care — bringing together providers, specialists, community resources, and the member to support coordinated, whole-person care.

Remote Work Environment

This position is designed for remote virtual care coordination delivery.
