Remote Telehealth LPN

Support telehealth and digital care programs across a broad range of virtual clinical functions — from patient outreach to visit support to clinical coordination — from home.

$25–$39 / hour 100% Remote Full-Time / Part-Time Multiple U.S. States

Job Overview

Virelia Health connects healthcare professionals with opportunities across telehealth, virtual care, and digital health programs.

We are seeking qualified LVNs/LPNs for remote telehealth opportunities supporting health systems, telehealth companies, virtual-first clinics, and payer-sponsored digital health programs.

A Remote Telehealth LPN supports telehealth programs across a broad range of virtual clinical functions — from pre-visit patient outreach and technology support, to in-visit assistance for virtual clinicians, to post-visit follow-up and documentation. Rather than being tied to a single telehealth sub-specialty (triage, virtual care, or video visits), this role provides flexible coverage across the full telehealth delivery workflow.

This position is well-suited to an LVN who enjoys variety, works comfortably across digital platforms, and wants a foundation in the fastest-growing segment of healthcare delivery.

The specific telehealth program, patient population, technology stack, and scope of responsibilities will depend on the employer and assignment.

This opportunity may be suitable for professionals searching for remote telehealth LPN jobs, remote telehealth nurse jobs, work-from-home telehealth LPN, remote digital health nurse, or virtual nurse LPN jobs roles.

Telehealth Roles Fit Every Schedule — You Choose Your Coverage

Telehealth LPN roles are some of the most schedule-flexible positions in remote nursing, because telehealth programs run across every hour of the day, every day of the week. Depending on the program, you may work traditional weekday hours, extended evening coverage, weekend shifts, or compressed 3- or 4-day weeks with longer blocks. Some telehealth programs even support microshifts (4–6 hours) — ideal for part-time work or supplemental income. Discuss your preferred hours during your interview.

What You'll Do

As a Remote Telehealth LPN, your responsibilities may span the full telehealth delivery workflow depending on the assignment.

  • Contact patients before virtual visits to confirm appointments, verify technology, and answer setup questions.
  • Support virtual clinicians during telehealth encounters by reviewing charts, preparing documentation, and coordinating next steps.
  • Conduct post-visit follow-up with patients to reinforce care instructions and confirm that next steps are completed.
  • Answer routine patient questions through secure messaging and patient portals within the LPN/LVN scope.
  • Coordinate referrals, labs, imaging, and follow-up appointments for virtual care patients.
  • Document all virtual interactions accurately in the telehealth platform and EHR.
  • Identify barriers to virtual care participation (technology, connectivity, health literacy) and coordinate support.
  • Deliver approved patient education on chronic conditions, medications, and self-management.
  • Escalate clinical concerns, new symptoms, or care-plan changes to an RN or provider.
  • Support preventive care scheduling and annual wellness activities within telehealth programs.
  • Coordinate with internal teams (care management, pharmacy, member services) to ensure continuity of care.
  • Participate in quality improvement, calibration, and patient experience initiatives.
  • Protect patient privacy and follow HIPAA requirements across all digital touchpoints.

Telehealth Focus

Telehealth is the broadest category of virtual care — it includes triage, virtual visits, video consults, remote monitoring, and digital patient outreach, all delivered through technology rather than in-person encounters. Telehealth LPNs are the flexible clinical workforce behind these programs.

Depending on the program, telehealth LPN work may involve:

Pre-visit outreach
In-visit clinical support
Post-visit follow-up
Secure messaging
Virtual care navigation
Patient education

The LPN/LVN's role is to support virtual care delivery — not to independently diagnose, prescribe, or alter treatment. Clinical concerns that exceed the LPN/LVN scope are escalated to an RN or provider according to the employer's protocols.

This structure keeps the role focused on virtual patient support, coordination, documentation, and appropriate escalation across the full telehealth workflow.

Patient Outreach & Virtual Support

Telehealth LPNs spend much of their time connecting with patients through virtual channels — making sure they understand what's expected before, during, and after virtual visits.

You may help patients:

  • Prepare for upcoming virtual visits — including technology setup and questions to ask.
  • Understand their care instructions in plain language after virtual encounters.
  • Complete recommended labs, screenings, and follow-up appointments.
  • Navigate the virtual care system and use digital health tools confidently.
  • Recognize warning signs that require contacting their provider.
  • Coordinate logistics like transportation, interpreter services, or family involvement.

Clear communication and patient empathy are essential because telehealth delivery depends on patient engagement with digital tools.

Telehealth Workflow

A Remote Telehealth LPN may follow a structured workflow across the telehealth delivery cycle.

Depending on the employer's protocols, you may:

Pre-Visit Outreach Technology Check Visit Support Post-Visit Follow-Up Document Encounter Coordinate Care Escalate If Needed

Telehealth is Not Emergency Care

Telehealth supports — but does not replace — emergency medical services. If a patient presents with emergency symptoms during any virtual interaction, the nurse follows the employer's emergency escalation process immediately.

Required Qualifications

  • Current LPN or LVN license applicable to the specific assignment.
  • Nursing education appropriate for LPN/LVN practice.
  • Clinical nursing experience with exposure to telehealth, virtual care, or patient communication.
  • Comfortable working across multiple digital platforms (video, chat, EHR, portals).
  • Strong written and verbal communication skills.
  • Strong organizational and documentation skills.
  • Ability to reinforce care plans and approved patient education.
  • Ability to recognize situations requiring clinical escalation.
  • Ability to work independently in a remote environment.
  • Ability to maintain confidentiality when handling patient information.

A Typical Day

A typical day may begin by reviewing your assigned telehealth panel — upcoming virtual visits, patient messages, and outstanding follow-up tasks.

You may start with pre-visit outreach to patients scheduled later in the day, confirming their technology works and answering any pre-visit questions.

As visits happen, you may support virtual clinicians by reviewing charts, preparing documentation, or coordinating next steps in real time.

Between encounters, you'll respond to secure messages, schedule follow-up appointments, coordinate referrals, and document all patient interactions in the telehealth platform.

Any clinical concern that exceeds the LPN/LVN scope is escalated to an RN or provider.

The role combines patient outreach, virtual visit support, care coordination, documentation, patient education, and appropriate escalation.

Unlike single-function telehealth roles, this position offers variety — every day touches multiple parts of the virtual care workflow.

Remote Work Environment

This position is designed for remote telehealth delivery.

Depending on the employer, you may work with:

Telehealth visit platforms
Electronic health records
Patient portals
Secure messaging systems
Telephony systems
Scheduling and referral platforms

Candidates should be comfortable with high-volume virtual interactions and multiple digital systems.

Top-demand states: California, Texas, Florida, New York, Pennsylvania, Ohio, Illinois, Michigan, Georgia, North Carolina, Arizona, Colorado.

Strong secondary markets: Connecticut, Indiana, Kentucky, Maryland, Massachusetts, Minnesota, Missouri, New Jersey, Tennessee, Virginia, Washington, Wisconsin.
