Remote Virtual Primary Care Nurse

Support virtual primary care — preventive care support, chronic condition management, care plan reinforcement, and primary care coordination from home.

$32–$54 / hour 100% Remote Full-Time / Part-Time Multiple U.S. States

Job Overview

Virelia Health connects registered nurses with opportunities across virtual primary care, telehealth nursing, and digital primary care programs.

We are seeking qualified Virtual Primary Care Nurses for remote opportunities supporting virtual primary care companies, health systems, health plans, employer health programs, and digital primary care organizations.

A Remote Virtual Primary Care Nurse supports virtual primary care — preventive care support, chronic condition management, care plan reinforcement, and primary care coordination. Working within their state licensure and clinical scope of practice, the virtual primary care nurse supports patients through virtual primary care visits, chronic condition check-ins, preventive care outreach, and care coordination.

This position is well-suited to a registered nurse with primary care, ambulatory, chronic care management, or virtual care experience who wants to practice in a virtual setting — providing meaningful clinical primary care support with schedule flexibility and without the demands of a facility-based environment.

The specific virtual primary care program, patient population, clinical model, and scope of responsibilities will depend on the employer and assignment.

This opportunity may be suitable for professionals searching for remote virtual primary care nurse jobs, work-from-home primary care RN jobs, remote primary care nursing jobs, virtual primary care RN, or remote chronic care RN roles.

Scheduled Virtual Primary Care with Predictable Hours

Virtual primary care nursing runs on scheduled patient visits and outreach blocks, giving your clinical day predictability and structure. Programs typically offer daytime, evening, or weekend coverage to accommodate patients who work or have family commitments. Many programs offer flexible 8- or 10-hour shifts, and some RNs work 4-day compressed weeks or part-time schedules. There are no facility floor assignments and no physical demands. We offer both full-time and part-time positions. Discuss your preferred schedule during your interview.

What You'll Do

As a Remote Virtual Primary Care Nurse, you may support patients across a range of virtual primary care encounters.

  • Support virtual primary care visits alongside providers — pre-visit prep, visit support, and post-visit follow-up.
  • Provide RN-led virtual primary care support for chronic condition check-ins and preventive care outreach.
  • Support patients with chronic conditions — diabetes, hypertension, asthma, COPD, and heart failure.
  • Reinforce provider care plans and clinical instructions.
  • Provide patient education on conditions, medications, self-management, and lifestyle modification.
  • Support patients with medication understanding and adherence.
  • Support preventive care outreach — screenings, immunizations, and annual wellness visits.
  • Coordinate care across providers, specialists, and community resources.
  • Document each patient encounter accurately in the EHR.
  • Identify patients with rising risk or unmet needs and escalate to the appropriate clinical team.
  • Support patients through care transitions — post-discharge, post-ED, and specialist referrals.
  • Coordinate with interdisciplinary care teams — providers, care managers, and specialists.
  • Participate in virtual primary care quality improvement, clinical review, and education.
  • Maintain current knowledge of primary care guidelines, chronic condition management, and licensure requirements.
  • Protect patient privacy and maintain HIPAA compliance across all virtual care interactions.

Virtual Primary Care Nursing Focus

Virtual primary care is a growing area of clinical practice — driven by increased patient demand for virtual care, payer coverage for telehealth, and health system investment in digital primary care delivery. Virtual primary care nurses extend clinical reach, improve access to primary care, and support patients with chronic conditions and preventive care needs.

Depending on the program, virtual primary care nursing may focus on:

Virtual primary care visits
Chronic condition management
Preventive care support
Medication support
Care coordination
Patient education

The virtual primary care nurse's role is to provide virtual primary care nursing within their licensure and scope of practice — including assessment, chronic condition support, preventive care outreach, patient education, care plan reinforcement, documentation, and care coordination. Clinical decisions, medication changes, and diagnostic decisions belong to the treating providers; the virtual primary care nurse supports the patient's primary care and coordinates with the care team.

This structure keeps the role focused on chronic condition management, preventive care support, care coordination, documentation, and appropriate clinical escalation.

Chronic Condition Management & Preventive Care

Effective virtual primary care nursing depends on strong chronic condition management and preventive care support — the RN's clinical judgment and patient education skills determine how to help each patient achieve their health goals.

Depending on the program and clinical focus, you may support patients by:

  • Supporting patients with chronic conditions — diabetes, hypertension, asthma, COPD, and heart failure.
  • Reinforcing provider care plans — medication schedules, monitoring, and lifestyle modification.
  • Providing patient education on conditions, medications, and self-management.
  • Supporting medication understanding and adherence.
  • Supporting preventive care outreach — screenings, immunizations, and annual wellness visits.
  • Supporting patients with lifestyle modification — nutrition, activity, and smoking cessation.
  • Coordinating care across providers, specialists, and community resources.
  • Documenting each patient encounter accurately and consistently.
  • Escalating concerning clinical findings to the appropriate clinical team.
  • Following crisis escalation protocols when patients report urgent clinical symptoms.

Clinical judgment, patient education skill, and consistency are essential — virtual primary care nursing requires a strong patient-relationship orientation and the ability to help patients make sustainable health changes.

Virtual Primary Care Workflow

A Remote Virtual Primary Care Nurse may follow a structured workflow across each virtual clinical encounter.

Depending on the employer's protocols, you may:

Review Patient Panel Virtual Visit / Outreach Assess Patient Educate & Support Coordinate Care Document Encounter Escalate If Needed

Clinical Escalation & Patient Safety

If a patient reports acute clinical symptoms, urgent safety concerns, or other urgent needs during any virtual interaction, the RN must follow the employer's clinical escalation protocol immediately — including directing the patient to call 911, contact their provider, or seek emergency care as appropriate. Virtual care does not replace emergency care.

Required Qualifications

  • Active RN license in good standing in the state(s) required by the employer.
  • Previous clinical experience in primary care, ambulatory care, chronic care management, or virtual care nursing.
  • Strong clinical assessment, chronic condition management, and patient education skills.
  • Comfortable with telehealth platforms, EHR systems, and virtual primary care workflows.
  • Strong documentation and time-management skills.
  • Ability to recognize situations requiring clinical escalation.
  • Ability to work independently in a remote environment.
  • Ability to maintain confidentiality and HIPAA compliance.
  • Reliable internet suitable for real-time video and a private workspace suitable for confidential patient conversations.

A Typical Day

A typical day may begin by reviewing your virtual primary care panel — checking patient names, recent clinical notes, upcoming visits, and any chronic condition outreach that is due.

You'll support virtual primary care visits alongside providers — pre-visit prep, visit support, and post-visit follow-up. Depending on the program, you may also provide independent RN-led virtual primary care support for chronic condition check-ins and preventive care outreach.

Between encounters, you'll provide patient education, reinforce care plans, coordinate care with providers and specialists, and document each patient encounter accurately.

Throughout the day, you'll support patients with chronic conditions, conduct preventive care outreach, escalate concerning clinical findings to the appropriate clinical team, and follow up with patients who need extra support.

The role combines virtual primary care support, chronic condition management, preventive care outreach, patient education, documentation, and appropriate clinical escalation.

Unlike specialty virtual nursing, virtual primary care nursing focuses on the whole patient — chronic conditions, preventive care, and long-term health goals. It's a role that requires both strong clinical judgment and a patient-relationship orientation.
