Remote LVN Care Coordinator

Help patients navigate appointments, referrals, follow-up, and care-team communication — from the comfort of your home.

$25–$36 / hour 100% Remote Full-Time / Part-Time Multiple U.S. States

Job Overview

Virelia Health is connecting qualified healthcare professionals with opportunities across nursing, clinical care, telehealth, and healthcare operations.

We are seeking a Remote LVN Care Coordinator to support patients throughout their healthcare journey by helping organize services, coordinate follow-up, communicate with patients and care teams, and ensure important healthcare-related tasks are completed.

This role is particularly suited to an LVN who enjoys the organizational and patient-service side of healthcare. Rather than focusing primarily on bedside care, the Remote LVN Care Coordinator helps patients understand their next steps, coordinates appointments and referrals, follows up on outstanding needs, and keeps patient information accurately documented.

The position may involve communication with patients, providers, clinics, specialists, insurance-related contacts, and other members of the healthcare team.

This is a remote healthcare coordination opportunity available in multiple eligible U.S. states, subject to the licensing and geographic requirements of the specific assignment.

This opportunity may be suitable for professionals searching for remote LVN care coordinator jobs, remote LVN patient care coordinator, work-from-home LVN care coordinator, remote vocational nurse care coordination, or remote LVN healthcare navigation roles.

Flexible Scheduling Available

We understand that everyone's situation is different. That's why we offer both full-time and part-time positions with flexible hours. Whether you're looking for a standard 40-hour week, need to work around family commitments, want to supplement another job, or simply prefer a lighter schedule, we can accommodate your needs. Discuss your preferred schedule during your interview.
