Remote LVN Patient Monitoring

Help healthcare teams stay informed between visits — reviewing monitoring data, supporting patient outreach, and escalating concerns from home.

$25–$37 / hour 100% Remote Full-Time / Part-Time Multiple U.S. States

Job Overview

Virelia Health connects healthcare professionals with opportunities across nursing, clinical care, telehealth, and healthcare operations.

We are seeking qualified LVNs/LPNs for remote patient monitoring opportunities supporting patients through technology-enabled healthcare programs.

A Remote LVN Patient Monitoring professional helps healthcare teams stay informed about patients between traditional healthcare visits. Depending on the assignment, the role may involve reviewing patient-reported information, monitoring data received through approved remote-monitoring systems, communicating with patients, documenting observations, and escalating concerns according to established clinical protocols.

This position is different from traditional bedside nursing because much of the work takes place through digital monitoring systems, telephone communication, patient outreach, and electronic documentation.

The specific patient population, technology, monitoring requirements, and scope of responsibilities will depend on the employer and assignment.

This opportunity may be suitable for professionals searching for remote LVN patient monitoring jobs, remote LPN patient monitoring, remote LVN RPM nurse, remote LVN telehealth jobs, work-from-home LVN nursing, or remote LVN digital health roles.

Flexible Scheduling Available

We understand that everyone's situation is different. That's why we offer both full-time and part-time positions with flexible hours. Whether you're looking for a standard 40-hour week, need to work around family commitments, want to supplement another job, or simply prefer a lighter schedule, we can accommodate your needs. Discuss your preferred schedule during your interview.

What You'll Do

As a Remote LVN Patient Monitoring professional, you may help monitor patients participating in remote healthcare programs.

  • Review patient information and monitoring data received through approved healthcare systems.
  • Monitor assigned patient information according to established workflows and protocols.
  • Identify readings, alerts, or patient-reported concerns that require attention.
  • Contact patients when follow-up communication is required.
  • Confirm that patients understand basic monitoring procedures and communication expectations.
  • Help patients troubleshoot routine issues with approved monitoring processes when within the scope of the assignment.
  • Ask appropriate follow-up questions when patient information requires clarification.
  • Document patient communications, monitoring activities, observations, and follow-up actions accurately.
  • Communicate relevant information to the appropriate RN, provider, or clinical team member.
  • Escalate abnormal or concerning information according to established protocols.
  • Follow up with patients who have missed expected monitoring submissions when required.
  • Encourage appropriate participation in assigned remote-monitoring programs.
  • Help identify barriers that may prevent patients from successfully participating in remote monitoring.
  • Maintain accurate electronic patient records.
  • Follow organizational policies regarding privacy, documentation, patient communication, and clinical escalation.
  • Participate in team meetings, training, quality initiatives, and workflow improvement activities as required.

Remote Patient Monitoring Focus

Remote patient monitoring allows healthcare teams to receive information from patients while they are outside a traditional clinical setting.

Depending on the specific program, monitored information may include data associated with conditions such as:

Blood pressure
Blood glucose
Weight
Heart rate
Oxygen saturation
Other patient-reported measurements

The LVN's role is not to independently diagnose a patient based solely on a monitoring value. Instead, the professional follows the employer's established workflow, documents relevant information, communicates with the appropriate clinical team, and escalates concerns according to the assignment's protocols.

This distinction helps keep the position focused on monitoring, patient engagement, documentation, and appropriate clinical escalation.

Patient Outreach & Engagement

Patient participation is an important part of remote monitoring.

You may communicate with patients to:

  • Confirm that monitoring information has been received.
  • Follow up when expected information has not been submitted.
  • Answer routine questions within your role.
  • Reinforce approved monitoring instructions.
  • Help patients understand when and how to communicate concerns.
  • Identify difficulties using monitoring technology or completing assigned activities.
  • Escalate patient-reported symptoms or concerns through the appropriate clinical process.
  • Document each relevant interaction.

Strong communication and active listening are important because remote monitoring often depends on patients accurately communicating information from their home environment.

Monitoring & Escalation

A Remote LVN Patient Monitoring position may require attention to changes in patient information or monitoring alerts.

Depending on the employer's protocols, you may:

Review Monitoring Info Identify Follow-Up Contact Patient Gather Information Document Escalate to Clinical Team Complete Follow-Up Docs

Escalation Protocol

The exact escalation process will be defined by the employer and should always be followed.

Required Qualifications

  • Current LPN or LVN license applicable to the specific assignment.
  • Nursing education appropriate for LPN/LVN practice.
  • Previous healthcare or nursing experience.
  • Strong verbal and written communication skills.
  • Ability to communicate professionally with patients remotely.
  • Comfortable using computers and electronic healthcare systems.
  • Ability to accurately document patient interactions and monitoring activities.
  • Strong attention to detail.
  • Ability to follow established clinical workflows and protocols.
  • Ability to recognize situations requiring escalation.
  • Strong organizational and time-management skills.
  • Ability to work independently in a remote environment.
  • Ability to maintain confidentiality when handling patient information.

A Typical Day

A typical day may begin by reviewing assigned patients and checking the remote-monitoring platform for new information, alerts, or outstanding monitoring activities.

You may then follow up with patients who require communication, clarify information, document interactions, and communicate relevant findings to members of the clinical team.

Some patients may need assistance participating consistently in a monitoring program. In those situations, you may help identify barriers, provide approved guidance, and document the outcome.

When monitoring information or patient-reported symptoms require additional clinical attention, you will follow the employer's escalation process.

The role therefore combines nursing knowledge, technology, patient communication, monitoring, documentation, and clinical teamwork.

Remote Work Environment

This position is designed for remote healthcare delivery and monitoring.

Depending on the employer, you may work with:

Remote patient monitoring platforms
Electronic health records
Patient portals
Telephone systems
Secure messaging platforms
Telehealth software
Clinical dashboards
Digital documentation systems

Candidates should be comfortable learning new healthcare technology and working with multiple digital systems.

Because patient information may be involved, candidates should have an appropriate professional environment for confidential healthcare communication and documentation.

Specific technology, equipment, connectivity, working hours, and monitoring systems will vary by employer.

Licensure & State Eligibility

Remote nursing positions can involve state-specific licensing requirements because patients may be located in different states.

Applicants with an active LPN or LVN license, or an eligible multistate license where applicable, may apply.

Final eligibility depends on the states in which the employer's patients are located, the employer's requirements, and the licensing rules applicable to the specific position.

Candidates should confirm licensing eligibility for the individual assignment before accepting an offer.

Medical insurance
Dental coverage
Vision coverage
Paid time off
Retirement savings options
Professional development
Training opportunities
Employee assistance programs
Flexible scheduling
Remote-work arrangements

Eligibility, waiting periods, and specific benefit terms are confirmed with the employer during the offer stage.
