Remote Substance Abuse Counselor

Support patients in recovery through virtual counseling — conducting sessions, developing treatment plans, and providing consistent clinical care from home.

$22–$38 / hour 100% Remote Full-Time / Part-Time Multiple U.S. States

Job Overview

Virelia Health connects healthcare professionals with opportunities across virtual addiction treatment, telehealth counseling, and digital behavioral health programs.

We are seeking qualified Substance Abuse Counselors for remote opportunities supporting telehealth addiction treatment providers, virtual behavioral health companies, recovery support programs, and outpatient treatment centers.

A Remote Substance Abuse Counselor supports patients in recovery through virtual counseling — conducting individual and group sessions, developing and updating treatment plans, documenting clinical encounters, coordinating with clinical teams, and providing consistent clinical care. Working within their state licensure and scope of practice, the counselor helps patients build the skills, supports, and motivation they need to sustain recovery.

This position is well-suited to a licensed or certified addiction counselor who wants to practice in a virtual setting — providing meaningful clinical work without the demands of an inpatient or facility-based environment.

The specific treatment program, patient population, technology platform, and scope of responsibilities will depend on the employer and assignment.

This opportunity may be suitable for professionals searching for remote substance abuse counselor jobs, virtual addiction counselor, work-from-home addiction counselor, telehealth substance abuse counselor, or remote SUD counseling jobs roles.

Scheduled Session Blocks with Predictable Clinical Days

Virtual addiction counseling runs on scheduled patient sessions, giving your clinical day predictability and structure. Programs typically offer 45–60 minute session blocks, with options for daytime, evening, or weekend coverage to accommodate patients who work or have family commitments. Many teams offer flexible 6-, 8-, or 10-hour blocks, and some counselors work 4-day compressed weeks. We offer both full-time and part-time positions. Discuss your preferred schedule during your interview.

What You'll Do

As a Remote Substance Abuse Counselor, you may support patients across a range of virtual clinical encounters.

  • Conduct individual counseling sessions with patients via video or phone.
  • Lead or co-facilitate virtual group counseling sessions when required by the program.
  • Complete biopsychosocial and substance use assessments for new patients.
  • Develop, update, and document individualized treatment plans based on clinical assessment.
  • Apply evidence-based modalities such as motivational interviewing, CBT, and relapse prevention strategies.
  • Support patients through early recovery, ongoing recovery, and post-treatment phases.
  • Coordinate with clinical teams, medical providers, and recovery support services.
  • Document each session and clinical contact accurately in the behavioral health EHR.
  • Provide crisis assessment and safety planning when needed, following program protocol.
  • Escalate clinical emergencies, safety concerns, or acute risk to appropriate emergency services immediately.
  • Coordinate care with referral sources, primary care providers, and community resources.
  • Support patient engagement and follow-up for patients at risk of disengagement.
  • Maintain licensure-appropriate scope of practice and confidentiality at all times.
  • Participate in clinical supervision, case reviews, and quality improvement activities.
  • Protect patient privacy and maintain HIPAA compliance across all virtual interactions.

Virtual Addiction Counseling Focus

Virtual addiction counseling is one of the fastest-growing areas of behavioral health — driven by increased demand for substance use disorder treatment, payer coverage for telehealth, and patient preference for receiving care in a private, home-based setting.

Depending on the program, virtual addiction counseling may focus on:

Substance use disorder treatment
Early recovery support
Relapse prevention
Co-occurring mental health
Family and support systems
Aftercare planning

The counselor's role is to provide clinical counseling, treatment planning, documentation, and care coordination within their licensure and scope of practice. Clinical emergencies, safety concerns, and acute risk are escalated to emergency services or the appropriate clinical team immediately, following program protocol and applicable law.

This structure keeps the role focused on clinical counseling, treatment planning, documentation, care coordination, and appropriate escalation.

Therapeutic Approaches & Evidence-Based Care

Effective addiction counseling draws on a range of evidence-based approaches — the counselor's clinical judgment determines how to combine them to meet each patient's needs.

Depending on the program and your licensure, you may use:

  • Motivational interviewing to build readiness and resolve ambivalence about change.
  • Cognitive behavioral therapy (CBT) to identify and reframe thoughts that drive substance use.
  • Relapse prevention strategies to anticipate triggers and build coping plans.
  • Contingency management principles where applicable to reinforce recovery behaviors.
  • Twelve-step facilitation or other recovery-model support where aligned with the patient's preferences.
  • Trauma-informed care to support patients with co-occurring trauma histories.
  • Harm reduction approaches where appropriate to reduce risks associated with use.
  • Family and couples work where the program supports it and the patient consents.

Clinical judgment, empathy, and consistency are essential — recovery is often nonlinear, and the counselor's steady presence is often what helps patients stay engaged through difficult periods.

Virtual Counseling Workflow

A Remote Substance Abuse Counselor may follow a structured workflow across each patient relationship.

Depending on the employer's protocols, you may:

Review Patient Schedule Individual Session Assess & Plan Intervene Document Encounter Coordinate Care Crisis / Escalate If Needed

Crisis & Safety Protocols

If a patient presents with acute safety concerns — including suicidal ideation, overdose risk, or a medical emergency — the counselor must follow the employer's crisis escalation protocol immediately, including coordinating emergency services or a warm handoff as directed. Virtual counseling does not replace emergency care.

Required Qualifications

  • Current state counseling license or certification applicable to the specific assignment (LADC, CADC, LCADC, LMHC, LPC, LCSW, or equivalent).
  • Experience in substance use disorder treatment, behavioral health counseling, or recovery support.
  • Strong clinical documentation and treatment planning skills.
  • Strong patient engagement and therapeutic communication skills.
  • Ability to provide crisis assessment and safety planning within scope.
  • Comfortable with virtual care platforms and behavioral health documentation systems.
  • Ability to recognize situations requiring clinical escalation.

  • Ability to maintain confidentiality and HIPAA compliance.
  • Reliable internet and a private workspace suitable for confidential clinical sessions.

A Typical Day

A typical day may begin by reviewing your virtual session schedule — checking patient names, recent session notes, and any documentation that needs completion.

You'll conduct individual counseling sessions by video or phone, applying evidence-based approaches to support each patient's recovery goals. Depending on the program, you may also lead a virtual group session.

Between sessions, you'll document clinical encounters accurately, update treatment plans as patients progress, coordinate with clinical teams, and follow up with patients who need extra support.

Throughout the day, you'll provide crisis assessment and safety planning when needed, and escalate any acute concern to emergency services or the appropriate clinical team per protocol.

The role combines clinical counseling, treatment planning, documentation, care coordination, crisis assessment, and appropriate escalation.

Unlike in-person addiction counseling, virtual counseling relies on clear therapeutic communication across a screen — a different clinical skill set than facility-based work, and one that many patients now prefer.
