Remote Chronic Care Health Coach

Guide patients managing chronic conditions through behavior change — supporting lifestyle, motivation, and adherence through structured virtual coaching.

$20–$32 / hour 100% Remote Full-Time / Part-Time Multiple U.S. States

Job Overview

Virelia Health connects healthcare professionals with opportunities across virtual health coaching, chronic condition support, and digital behavior-change programs.

We are seeking qualified Chronic Care Health Coaches for remote opportunities supporting health plans, value-based care organizations, digital chronic care companies, and employer wellness programs.

A Remote Chronic Care Health Coach supports patients managing chronic conditions — such as diabetes, hypertension, prediabetes, obesity, and heart disease — by guiding them through structured behavior change, lifestyle modifications, and adherence support. Working within established coaching protocols, the health coach helps patients set goals, build sustainable habits, and stay motivated over time.

This is a coaching role — not a clinical role. Health coaches do not provide clinical advice, adjust medications, interpret monitoring data diagnostically, or triage symptoms. Their focus is on supporting the behavior-change side of chronic condition management — the daily habits and choices that influence long-term outcomes.

This position is well-suited to someone with health coaching, patient education, or chronic condition support experience who wants to help patients build healthier lives through structured coaching relationships.

The specific coaching program, patient population, technology platform, and scope of responsibilities will depend on the employer and assignment.

This opportunity may be suitable for professionals searching for remote chronic care health coach jobs, virtual health coach, work-from-home chronic care coach, remote diabetes health coach, or remote lifestyle coaching jobs roles.

Consistent Coaching Cadence with Predictable Sessions

Health coaching programs run on scheduled session cadences — typically weekly or bi-weekly calls with each patient on your panel. This gives your day structure and predictability, with regular session blocks, steady patient rosters, and clear start and end times. Most programs run weekday business hours with occasional extended-hour options, and many coaches work 4-day or compressed weeks. There are no overnight shifts, no emergency response, and no clinical decision pressure. We offer both full-time and part-time positions. Discuss your preferred schedule during your interview.

What You'll Do

As a Remote Chronic Care Health Coach, you may support patients across a structured coaching program.

  • Conduct scheduled virtual coaching sessions with patients by phone or video.
  • Build supportive coaching relationships based on trust, empathy, and consistency.
  • Use motivational interviewing techniques to help patients identify their own goals and priorities.
  • Support patients in setting realistic, measurable goals for lifestyle and self-management.
  • Coach patients on nutrition, physical activity, sleep, stress management, and other lifestyle factors.
  • Support adherence to care plans and provider recommendations through encouragement and accountability.
  • Help patients identify and work through barriers to behavior change.
  • Reinforce education provided by the patient's clinical team — without providing clinical advice.
  • Document each coaching session accurately in the program platform.
  • Track patient progress toward goals and adjust coaching approaches over time.
  • Coordinate with clinical teams, care managers, and program staff as needed.
  • Escalate any clinical question, symptom, or medical concern to the appropriate clinical team per protocol.
  • Encourage ongoing patient engagement with the coaching program.
  • Participate in coaching team meetings, case reviews, and quality improvement activities.
  • Protect patient privacy and maintain HIPAA compliance across all coaching interactions.

Chronic Care Coaching Focus

Health coaching for chronic conditions is one of the fastest-growing areas of digital health — driven by payer investment in value-based care, employer wellness programs, and evidence that coaching improves outcomes for conditions like diabetes, hypertension, and prediabetes.

Depending on the program, chronic care coaching may focus on:

Diabetes and prediabetes support
Hypertension lifestyle coaching
Weight management
Nutrition coaching
Physical activity support
Stress and sleep support

The health coach's role is non-clinical — supporting behavior change, motivation, and adherence. Health coaches do not diagnose, prescribe, provide clinical advice, interpret medical data, or make treatment decisions. Any clinical question, symptom, or concern that requires clinical judgment is escalated to the appropriate clinical team immediately.

This structure keeps the role focused on coaching relationships, goal setting, lifestyle support, documentation, and appropriate escalation.

Motivational Interviewing & Behavior Change

Motivational interviewing and structured behavior-change techniques are the foundation of effective health coaching — the skills that help patients move from intention to sustained action.

You may help patients:

  • Identify what matters most to them and why change is worth pursuing.
  • Set goals that feel meaningful, achievable, and personally motivating.
  • Break larger goals into small, sustainable steps.
  • Recognize patterns that get in the way of progress — without judgment.
  • Build confidence through small wins and reflection.
  • Develop self-management skills they can maintain long-term.
  • Return to their plan after setbacks without abandoning it entirely.
  • Celebrate progress and stay engaged with their health over time.

Empathy, patience, and structured coaching skills are essential — behavior change is hard, and the coach's steady, non-judgmental presence is what makes sustained progress possible.

Health Coach Workflow

A Remote Chronic Care Health Coach may follow a structured workflow across each patient relationship.

Depending on the employer's protocols, you may:

Review Patient Roster Scheduled Session Goal Check-In Behavior Coaching Reinforce Plan Document Session Escalate If Needed

Non-Clinical Scope & Escalation

Health coaches operate within a clearly non-clinical scope. Any patient question, symptom, or concern that requires clinical judgment — including medication questions, symptom interpretation, or medical advice — is escalated to the appropriate clinical team per the employer's protocol. Coaching never replaces clinical care.

Required Qualifications

  • Previous experience in health coaching, patient education, chronic condition support, or behavioral health support.
  • Strong motivational interviewing and coaching communication skills.
  • Comfortable with digital coaching platforms and documentation systems.
  • Ability to follow structured coaching protocols consistently.
  • Strong documentation skills and attention to detail.
  • Ability to recognize situations requiring clinical escalation.
  • Ability to work independently in a remote environment.
  • Ability to maintain confidentiality and HIPAA compliance.
  • Reliable internet and a private workspace suitable for confidential patient conversations.

A Typical Day

A typical day may begin by reviewing your patient roster — checking which patients have scheduled sessions, who has upcoming goal check-ins, and who has recently started or completed a coaching module.

You'll connect with patients by phone or video for scheduled sessions, using motivational interviewing to explore progress, setbacks, and what feels most important to them next.

Between sessions, you'll document your coaching notes, coordinate with clinical teams on any escalated concerns, and follow up with patients who have missed sessions or need extra support.

The role combines coaching sessions, behavior-change support, goal tracking, documentation, and appropriate escalation.

Unlike clinical chronic care roles, health coaching is focused on the person's daily life and habits — supporting the choices and behaviors that shape how they feel over time.
