Remote Occupational Therapy Assistant

Coach patients through daily living activities, adaptive strategies, and functional recovery — helping them regain independence from home.

$24–$36 / hour 100% Remote Full-Time / Part-Time Multiple U.S. States

Job Overview

Virelia Health connects healthcare professionals with opportunities across virtual occupational therapy, telehealth rehabilitation, and digital functional-care programs.

We are seeking qualified Occupational Therapy Assistants (OTAs) for remote rehabilitation opportunities supporting virtual OT clinics, telehealth functional-recovery programs, and digital rehab platforms.

A Remote Occupational Therapy Assistant supports patients participating in virtual occupational therapy programs by coaching them through activities of daily living (ADLs), reinforcing adaptive strategies and equipment use, observing functional progress, and documenting each encounter. Working under the direction of a licensed Occupational Therapist, the OTA helps patients rebuild independence in everyday tasks — from dressing and meal preparation to fine motor coordination and home safety.

This position is well-suited to an OTA who enjoys patient coaching, functional problem-solving, and helping patients achieve practical daily-life goals through virtual care.

The specific virtual OT program, patient population, technology platform, and scope of responsibilities will depend on the employer and assignment.

This opportunity may be suitable for professionals searching for remote occupational therapy assistant jobs, virtual OTA jobs, work-from-home occupational therapy assistant, telehealth OT assistant, or remote ADL coaching OTA roles.

Session-Based Schedule with Functional-Focused Days

Virtual OT programs are built around scheduled patient sessions, giving your day predictable start and end times. Most programs offer flexible session blocks (30–60 minutes) during weekday hours, with some early morning or evening coverage for patients who work or have family responsibilities. There are no overnight shifts, no weekend rotations in most programs, and no on-call. We offer both full-time and part-time positions, and many virtual OTAs work 4-day compressed weeks. Discuss your preferred schedule during your interview.

What You'll Do

As a Remote Occupational Therapy Assistant, you may support virtual OT patients across scheduled sessions and digital touchpoints.

  • Conduct virtual OT sessions with patients via video, following the supervising occupational therapist's plan of care.
  • Coach patients through activities of daily living (ADLs) such as dressing, grooming, bathing, and meal preparation.
  • Guide patients through instrumental activities of daily living (IADLs) such as cooking, shopping, home management, and medication routines.
  • Reinforce adaptive equipment use and compensatory strategies as prescribed by the OT.
  • Observe and document patients' functional progress, tolerance, and any changes in ability.
  • Provide structured cognitive and fine motor activities as directed by the supervising OT.
  • Support home safety education — identifying fall risks, recommending environmental modifications, and discussing safe mobility.
  • Document each session accurately in the rehabilitation EHR, including activities performed and patient response.
  • Communicate relevant patient observations to the supervising occupational therapist.
  • Escalate changes in patient condition, new safety concerns, or declining function to the supervising OT immediately.
  • Support patient adherence to home programs through outreach and encouragement.
  • Help patients troubleshoot technology issues with the virtual OT platform.
  • Coordinate scheduling and follow-up sessions with the care team.
  • Participate in team meetings, case reviews, and quality improvement activities.
  • Protect patient privacy and maintain HIPAA compliance across all virtual sessions.

Virtual Occupational Therapy Focus

Virtual occupational therapy helps patients regain independence in everyday activities that matter to them. Unlike physical therapy — which focuses on movement and strength — OT focuses on function: how a patient performs real-world tasks safely and effectively.

Depending on the program, virtual OT may focus on:

Stroke recovery
Fine motor rehabilitation
Home safety and fall prevention
Post-surgical functional recovery
Cognitive rehabilitation
Pediatric functional therapy

The OTA's role is to carry out the supervising occupational therapist's plan of care, provide functional coaching, document objective findings, and escalate any concerns. OTAs do not independently evaluate, diagnose, or modify treatment plans — those functions belong to the supervising Occupational Therapist, and the OTA coordinates with them per state practice act requirements.

This structure keeps the role focused on functional activity coaching, adaptive strategy reinforcement, progress monitoring, documentation, and appropriate escalation.

Virtual ADL & Functional Coaching

Virtual OT requires excellent observational and verbal coaching skills — you're helping patients adapt real-life tasks without being physically present.

You may help patients:

  • Practice daily living tasks in their own home environment.
  • Adapt activities when physical limitations interfere.
  • Learn to use adaptive equipment correctly and confidently.
  • Modify their space for safety and efficiency.
  • Track progress on functional goals set by the OT.
  • Build confidence in tasks that feel difficult or overwhelming.
  • Know when a task is unsafe and needs to be stopped.
  • Stay motivated and consistent with their home program.

Clear demonstration, verbal cues, patience, and creativity are essential — virtual OT often involves adapting tasks to fit whatever the patient has available at home.

Virtual OT Workflow

A Remote Occupational Therapy Assistant may follow a structured workflow across each virtual session.

Depending on the employer's protocols, you may:

Review Plan of Care Session Prep Virtual OT Session Coach ADLs Document Encounter Coordinate with OT Escalate If Needed

Supervision Requirements

OTAs work under the supervision of a licensed Occupational Therapist per state practice acts and payer requirements. Supervision structure, co-signature requirements, and scope of duties vary by state and by employer. All sessions must remain within the OTA's authorized scope.

Required Qualifications

  • Current Occupational Therapy Assistant (OTA) license or certification applicable to the specific assignment.
  • Associate degree from an accredited OTA program.
  • Previous clinical experience in outpatient, rehabilitation, pediatric, or comparable OT settings.
  • Strong verbal communication and patient coaching skills.
  • Comfortable guiding patients through tasks via video.
  • Comfortable with digital documentation platforms and virtual care tools.
  • Strong documentation and time-management skills.
  • Ability to recognize situations requiring clinical escalation.
  • Ability to work independently in a remote environment.
  • Ability to maintain confidentiality and HIPAA compliance.

Experience in any of the following may be beneficial:

Stroke recovery
Post-surgical rehabilitation
Pediatric OT
Cognitive rehabilitation

A Typical Day

A typical day may begin by reviewing your virtual OT schedule — checking which patients have sessions, their current functional goals, and any notes from the supervising occupational therapist.

You'll connect with each patient at their scheduled time and guide them through activities from their plan of care — practicing dressing, meal preparation, fine motor tasks, or cognitive exercises.

Because you're working through video, you'll rely on clear verbal cues, demonstrations, and creativity to help patients adapt tasks to their home environment.

After each session, you'll document observations accurately and communicate any changes in function or safety concerns to the supervising OT.

The role combines functional activity coaching, adaptive strategy reinforcement, patient education, documentation, and appropriate escalation.

Unlike in-person OT clinics, virtual OT relies heavily on observation and verbal coaching — a different clinical skill set than hands-on care.

Depending on the employer, you may work with:
