Remote Family Therapist

Provide virtual family and couples therapy — supporting relationships, family systems, and communication through secure telehealth platforms from home.

$28–$48 / hour 100% Remote Full-Time / Part-Time Multiple U.S. States

Job Overview

Virelia Health connects healthcare professionals with opportunities across virtual family therapy, couples counseling, and digital behavioral health programs.

We are seeking qualified Family Therapists (LMFTs) for remote opportunities supporting telehealth therapy companies, virtual behavioral health platforms, health systems, and digital mental health programs.

A Remote Family Therapist provides virtual systemic therapy to couples and families through secure telehealth platforms. Working within their state licensure and clinical scope of practice, the family therapist delivers evidence-based relational therapy — supporting couples through relationship challenges, families through transitions and conflict, and individuals within the context of their relationships and family systems.

This position is well-suited to a Master's- or doctoral-level marriage and family therapist who wants to practice in a virtual setting — providing meaningful relational clinical work with schedule flexibility and without the demands of a facility-based environment.

The specific family therapy program, patient population, therapeutic modalities, and scope of responsibilities will depend on the employer and assignment.

This opportunity may be suitable for professionals searching for remote family therapist jobs, virtual marriage and family therapist, work-from-home LMFT, telehealth couples therapist, or remote relational therapy jobs roles.

Scheduled Relational Sessions with Flexible Coverage

Virtual family therapy runs on scheduled patient sessions, giving your clinical day predictability and structure. Programs typically offer 50–60 minute therapy sessions, with options for daytime, evening, or weekend coverage to accommodate couples and families who work or have children in school. Many platforms offer flexible 6-, 8-, or 10-hour blocks, and some therapists work 4-day compressed weeks or set their own caseload. We offer both full-time and part-time positions. Discuss your preferred schedule during your interview.

What You'll Do

As a Remote Family Therapist, you may support couples and families across a range of virtual clinical encounters.

  • Conduct virtual family, couples, and relational therapy sessions via video.
  • Complete systemic and relational assessments for new patients, couples, and families.
  • Develop, update, and document individualized treatment plans based on clinical assessment.
  • Apply evidence-based relational modalities such as EFT, Gottman Method, structural, narrative, and solution-focused therapy.
  • Support couples through communication, conflict, intimacy, and trust work.
  • Support families through transitions, parenting challenges, blended family dynamics, and intergenerational issues.
  • Work with individual clients on relational patterns and family-of-origin issues.
  • Provide crisis assessment, safety planning, and de-escalation when needed.
  • Coordinate care with psychiatrists, primary care providers, and community resources.
  • Document each session and clinical contact accurately in the behavioral health EHR.
  • Escalate clinical emergencies, safety concerns, or acute risk to emergency services immediately.
  • Coordinate care with referral sources and multidisciplinary teams.
  • Maintain licensure-appropriate scope of practice, clinical boundaries, and confidentiality.
  • Participate in clinical supervision, case reviews, and quality improvement activities.
  • Protect patient privacy and maintain HIPAA compliance across all virtual interactions.

Virtual Family & Couples Therapy Focus

Virtual family and couples therapy is one of the fastest-growing areas of behavioral health — driven by increased demand for relational services, payer coverage for telehealth, and patient preference for receiving care in a private, home-based setting.

Depending on the program, virtual family therapy may focus on:

Couples therapy
Family therapy
Parent-child relational work
Life transitions
Grief and loss
Blended family support

The family therapist's role is to provide systemic clinical services within their licensure and scope of practice — including assessment, treatment planning, relational therapy, and crisis intervention. Clinical emergencies, safety concerns, and acute risk are escalated to emergency services or the appropriate clinical team immediately, following program protocol and applicable law.

This structure keeps the role focused on systemic therapy, couples and family work, documentation, care coordination, and appropriate escalation.

Therapeutic Approaches & Evidence-Based Care

Effective family therapy draws on a range of evidence-based relational modalities — the LMFT's clinical judgment determines how to combine them to meet the needs of each couple or family.

Depending on the program and your training, you may use:

  • Emotionally focused therapy (EFT) to help partners rebuild attachment and connection.
  • Gottman Method to strengthen friendship, manage conflict, and build shared meaning.
  • Structural family therapy to clarify roles, boundaries, and family hierarchies.
  • Narrative therapy to help families rewrite the stories that shape their dynamics.
  • Solution-focused therapy to help couples and families move forward on concrete goals.
  • Bowen family systems to work with differentiation, triangles, and multigenerational patterns.
  • Cognitive behavioral couple therapy to address thoughts and behaviors that drive conflict.
  • Trauma-informed care as a foundational approach across all relational work.

Clinical judgment, empathy, and neutrality are essential — relational therapy requires the therapist to hold multiple perspectives at once and to help each member of the system feel heard.

Virtual Family Therapy Workflow

A Remote Family Therapist may follow a structured workflow across each patient or family relationship.

Depending on the employer's protocols, you may:

Review Session Schedule Systemic Assessment Treatment Planning Relational Intervention Document Encounter Coordinate Care Crisis / Escalate If Needed

Crisis & Safety Protocols

If any member of a couple or family presents with acute safety concerns — including suicidal ideation, self-harm, domestic violence, or a psychiatric emergency — the clinician must follow the employer's crisis escalation protocol immediately, including coordinating emergency services or a warm handoff as directed. Virtual therapy does not replace emergency care.

Required Qualifications

  • Current state LMFT license or equivalent marriage and family therapy license applicable to the specific assignment.
  • Master's or doctoral degree in marriage and family therapy or a related clinical field.
  • Clinical experience in couples therapy, family therapy, systemic therapy, or relational counseling.
  • Strong clinical documentation, treatment planning, and therapeutic communication skills.
  • Ability to provide crisis assessment and safety planning within scope.
  • Comfortable with telehealth platforms and behavioral health documentation systems.
  • Ability to recognize situations requiring clinical escalation.
  • Ability to work independently in a remote environment.
  • Ability to maintain confidentiality and HIPAA compliance.
  • Reliable internet and a private workspace suitable for confidential clinical sessions.

A Typical Day

A typical day may begin by reviewing your virtual session schedule — checking patient names, session notes, and any documentation that needs completion.

You'll conduct couples and family sessions by video, applying systemic approaches to support each couple or family's relational goals. Depending on the program, you may also conduct individual sessions for relational work, parent coaching, or child-focused therapy within a family systems frame.

Between sessions, you'll document clinical encounters accurately, update treatment plans as patients progress, coordinate care with clinical teams, and follow up with couples or families who need extra support.

Throughout the day, you'll provide crisis assessment and safety planning when needed, and escalate any acute concern to emergency services or the appropriate clinical team per protocol.

The role combines systemic therapy, relational assessment, treatment planning, documentation, case coordination, and appropriate escalation.

Unlike individual therapy, relational therapy requires holding multiple perspectives and facilitating communication between partners or family members — a distinct clinical skill set that virtual sessions can support well when the therapist is attentive to engagement across the screen.

Remote Work Environment

This position is designed for remote clinical delivery.

Depending on the employer, you may work with:

Telehealth platforms

Candidates should have a stable internet connection suitable for real-time video, a private workspace suitable for confidential clinical sessions, and comfort with multiple digital platforms.
