Remote Behavioral Health Utilization Review Nurse

Conduct psychiatric medical necessity reviews, determine behavioral health level of care, and support BH concurrent review — from home.

$36–$58 / hour 100% Remote Full-Time / Part-Time Multiple U.S. States

Job Overview

Virelia Health connects registered nurses with opportunities across behavioral health utilization review, virtual BH UR, and digital psychiatric review programs.

We are seeking qualified Behavioral Health Utilization Review Nurses for remote opportunities supporting health plans, managed care organizations (MCOs), utilization management companies, behavioral health organizations, and health systems.

A Remote Behavioral Health Utilization Review Nurse conducts psychiatric medical necessity reviews, determines behavioral health level of care, and supports BH concurrent review. Working within their state licensure and clinical scope of practice, the BH UR nurse reviews psychiatric and behavioral health clinical information against behavioral health criteria — MCG Behavioral Health, InterQual Behavioral Health, ASAM Criteria, or payer-specific criteria — to determine psychiatric medical necessity and appropriate level of care.

This position is well-suited to a registered nurse with psychiatric nursing, behavioral health nursing, utilization review, or case management experience who wants to practice in a virtual setting — providing meaningful BH clinical review work with schedule flexibility and without the demands of a facility-based environment.

The specific behavioral health UR program, plan type, criteria set, and scope of responsibilities will depend on the employer and assignment.

This opportunity may be suitable for professionals searching for remote behavioral health utilization review nurse jobs, work-from-home BH UR nurse jobs, remote psychiatric utilization review jobs, virtual BH UR nurse, or remote behavioral health utilization management RN roles.

Structured BH Clinical Review with Predictable Hours

Behavioral health utilization review runs on structured clinical review workflows — typically during weekday business hours, with some programs offering extended coverage. Many programs offer flexible 8- or 10-hour days, and some BH UR nurses work 4-day compressed weeks or part-time schedules. There are no facility floor assignments and no physical demands. We offer both full-time and part-time positions. Discuss your preferred schedule during your interview.

What You'll Do

As a Remote Behavioral Health Utilization Review Nurse, you may support health plans and MCOs across the full behavioral health utilization management lifecycle.

  • Review behavioral health clinical information against evidence-based criteria — MCG Behavioral Health, InterQual Behavioral Health, ASAM Criteria, or payer-specific criteria.
  • Determine psychiatric medical necessity for requested behavioral health services, admissions, and continued stays.
  • Determine appropriate behavioral health level of care — inpatient psychiatric, residential, partial hospitalization, intensive outpatient, or outpatient settings.
  • Conduct prospective review (pre-service) for behavioral health admissions and services.
  • Conduct concurrent review (during service) for ongoing psychiatric admissions and residential treatment.
  • Conduct retrospective review (post-service) for behavioral health services as assigned.
  • Apply behavioral health level-of-care criteria accurately and consistently.
  • Document clinical review decisions accurately in the utilization management platform.
  • Communicate with psychiatric providers and facilities on clinical review outcomes.
  • Support peer-to-peer review processes when behavioral health denials are escalated.
  • Coordinate with behavioral health medical directors on complex psychiatric reviews.
  • Support appeal reviews and second-level behavioral health clinical reviews as assigned.
  • Identify opportunities for behavioral health care optimization and alternative levels of care.
  • Maintain compliance with behavioral health parity regulations, CMS requirements, and state regulations.
  • Participate in behavioral health utilization management quality improvement and clinical review.
  • Maintain current knowledge of behavioral health clinical guidelines, criteria updates, and regulatory changes.
  • Protect member privacy and maintain HIPAA compliance across all behavioral health utilization review activities.

Behavioral Health Utilization Review Focus

Behavioral health utilization review is a growing area of psychiatric nursing — driven by payer investment in behavioral health utilization management, parity legislation requiring equal coverage for mental health and SUD, and the recognition that evidence-based psychiatric review ensures appropriate behavioral health care delivery. Behavioral health UR nurses extend clinical review reach, improve behavioral health care access, and support quality psychiatric care across the behavioral health continuum.

Depending on the program, behavioral health utilization review may focus on:

Psychiatric medical necessity
Behavioral health level of care
Psychiatric inpatient review
Residential treatment review
SUD treatment review
BH criteria application

The BH UR nurse's role is to provide behavioral health clinical review within their licensure and scope of practice — including psychiatric medical necessity review, behavioral health level-of-care determination, criteria application, documentation, and appropriate escalation. Clinical decisions, ultimate behavioral health medical necessity determinations, and clinical denial decisions belong to the behavioral health medical director or clinical leadership; the BH UR nurse supports the clinical review process and coordinates with behavioral health clinical leadership.

This structure keeps the role focused on behavioral health clinical review, criteria application, psychiatric medical necessity determination, documentation, and appropriate escalation.

BH Criteria Application & Psychiatric Medical Necessity Determination

Effective behavioral health utilization review depends on strong criteria application and psychiatric medical necessity determination — the RN's clinical judgment and behavioral health criteria knowledge determine the appropriate review outcome.

Depending on the program and clinical focus, you may support clinical review by:

  • Applying MCG Behavioral Health criteria accurately.
  • Applying InterQual Behavioral Health criteria accurately.
  • Applying ASAM Criteria for substance use disorder treatment accurately.
  • Applying payer-specific or plan-specific behavioral health criteria accurately.
  • Determining psychiatric medical necessity based on clinical documentation and criteria.
  • Determining appropriate behavioral health level of care based on clinical presentation and criteria.
  • Documenting clinical review decisions accurately and consistently.
  • Communicating clinical review outcomes to psychiatric providers and facilities.
  • Escalating behavioral health clinical denials or complex cases to the behavioral health medical director or clinical leadership.
  • Supporting peer-to-peer review processes when clinically appropriate.
  • Maintaining compliance with behavioral health parity and regulatory requirements.

Clinical judgment, behavioral health criteria knowledge, and clear communication are essential — behavioral health UR nurses are the clinical reviewers who apply behavioral health criteria to support appropriate psychiatric care delivery.

Behavioral Health Utilization Review Workflow

A Remote Behavioral Health Utilization Review Nurse may follow a structured workflow across each behavioral health clinical review.

Depending on the employer's protocols, you may:

Receive BH Review Request Review Psychiatric Clinical Info Apply BH Criteria Determine Level of Care Document Decision Communicate Outcome Escalate If Needed

Parity Compliance & Clinical Escalation

Behavioral health utilization review directly affects member access to behavioral health care, psychiatric provider reimbursement, and parity compliance. Reviews must be conducted in accordance with evidence-based behavioral health criteria, behavioral health parity regulations, CMS requirements, NCQA/URAC accreditation standards, and applicable state regulations. Behavioral health clinical denials and complex cases must be escalated to the behavioral health medical director or clinical leadership following the employer's escalation protocol.

Required Qualifications

  • Active RN license in good standing in the state(s) required by the employer.
  • Previous clinical experience in psychiatric nursing, behavioral health nursing, utilization review, or case management.
  • Working knowledge of MCG, InterQual, and behavioral health level-of-care criteria.
  • Strong psychiatric review, medical necessity determination, and clinical documentation skills.
  • Comfortable with utilization management platforms, EHR systems, and payer clinical review workflows.
  • Strong documentation and time-management skills.
  • Ability to recognize situations requiring clinical escalation.
  • Ability to work independently in a remote environment.

A Typical Day

A typical day may begin by logging in, checking your behavioral health utilization review queue, and prioritizing cases based on urgency, admission date, or review type.

You'll review behavioral health clinical information — psychiatric records, provider notes, and behavioral health documentation — against the appropriate behavioral health criteria (MCG, InterQual, ASAM, or payer-specific). You'll determine psychiatric medical necessity and behavioral health level of care, document your clinical review decision accurately, and communicate the review outcome to psychiatric providers and facilities.

Between reviews, you'll coordinate with the behavioral health medical director on complex psychiatric decisions, support peer-to-peer review processes, and follow up on escalated cases.

Throughout the day, you'll ensure compliance with behavioral health parity and regulatory requirements, maintain accurate documentation, and stay current on behavioral health criteria updates and psychiatric clinical guidelines.

The role combines psychiatric medical necessity review, behavioral health criteria application, level-of-care determination, clinical documentation, psychiatric provider communication, and appropriate clinical escalation.

Unlike general utilization review nursing, behavioral health utilization review focuses specifically on psychiatric and behavioral health criteria — you're the RN who applies behavioral health criteria to support appropriate psychiatric care delivery across the behavioral health continuum.
