Remote Prior Authorization RN

Review prior authorization requests for medical and pharmacy benefits — supporting clinical decision reviews for health plans and managed care from home.

$32–$52 / hour 100% Remote Full-Time / Part-Time Multiple U.S. States

Job Overview

Virelia Health connects registered nurses with opportunities across prior authorization, clinical review, and payer clinical operations.

We are seeking qualified Prior Authorization RNs for remote opportunities supporting health plans, managed care organizations (MCOs), pharmacy benefit managers (PBMs), utilization management companies, and healthcare systems.

A Remote Prior Authorization RN reviews prior authorization requests, applies clinical criteria, and supports medical and pharmacy benefit reviews for health plans and managed care organizations. Working within their state licensure and clinical scope of practice, the prior authorization RN reviews clinical information against payer criteria, medical policies, and clinical guidelines — determining whether requested services meet coverage requirements.

This position is well-suited to a registered nurse with prior authorization, utilization review, utilization management, case management, or acute care nursing experience who wants to practice in a virtual setting — providing meaningful clinical review work with schedule flexibility and without the demands of a facility-based environment.

The specific prior authorization program, benefit type (medical or pharmacy), criteria set, and scope of responsibilities will depend on the employer and assignment.

This opportunity may be suitable for professionals searching for remote prior authorization RN jobs, work-from-home PA nurse jobs, remote prior auth RN jobs, remote PA clinical review nurse, or remote pharmacy prior authorization nurse roles.

Structured Clinical Review with Predictable Hours

Prior authorization review runs on structured clinical review workflows — typically during weekday business hours, with some programs offering extended coverage. Many programs offer flexible 8- or 10-hour days, and some prior authorization RNs work 4-day compressed weeks or part-time schedules. There are no facility floor assignments and no physical demands. We offer both full-time and part-time positions. Discuss your preferred schedule during your interview.

What You'll Do

As a Remote Prior Authorization RN, you may support health plans and MCOs across the full prior authorization lifecycle.

  • Review prior authorization requests for medical and pharmacy benefits.
  • Review clinical information submitted by providers and facilities.
  • Apply payer medical policies, clinical criteria, and clinical guidelines accurately.
  • Apply MCG, InterQual, or payer-specific criteria as applicable.
  • Determine whether requested services meet coverage requirements based on clinical information and criteria.
  • Request additional clinical information from providers when documentation is incomplete.
  • Document clinical review decisions accurately in the prior authorization platform.
  • Communicate review outcomes to providers, members, and facilities.
  • Coordinate with medical directors and clinical leadership on complex reviews and denials.
  • Support peer-to-peer review processes when requested by providers.
  • Support member and provider inquiries on authorization status and requirements.
  • Maintain compliance with regulatory requirements — CMS, NCQA, URAC, state regulations, and turnaround time standards.
  • Participate in prior authorization quality improvement and clinical review.
  • Maintain current knowledge of medical policies, pharmacy policies, criteria updates, and regulatory changes.
  • Protect member privacy and maintain HIPAA compliance across all prior authorization activities.

Prior Authorization Focus

Prior authorization is a critical function in managed care — ensuring that requested medical and pharmacy services meet coverage requirements based on clinical criteria and medical policy. Prior authorization RNs are the clinical reviewers who apply criteria to support appropriate access to care.

Depending on the program, prior authorization review may focus on:

Medical benefit prior auth
Pharmacy benefit prior auth
Inpatient admissions
Surgical procedures
Advanced imaging
Specialty drugs

The prior authorization RN's role is to provide clinical review within their licensure and scope of practice — including clinical information review, criteria application, coverage determination support, documentation, and appropriate escalation. Clinical decisions, ultimate medical necessity determinations, and clinical denial decisions belong to the medical director or clinical leadership; the prior authorization RN supports the clinical review process and coordinates with clinical leadership.

This structure keeps the role focused on clinical review, criteria application, coverage determination support, documentation, and appropriate escalation.

Medical & Pharmacy Benefit Review

Effective prior authorization review depends on strong criteria application and benefit knowledge — the RN's clinical judgment and criteria knowledge determine the appropriate review outcome.

Depending on the program and clinical focus, you may support clinical review by:

  • Reviewing medical benefit prior authorizations — surgeries, procedures, imaging, and inpatient admissions.
  • Reviewing pharmacy benefit prior authorizations — specialty drugs, high-cost medications, and formulary exceptions.
  • Applying payer medical policies and clinical guidelines accurately.
  • Applying MCG, InterQual, or payer-specific criteria accurately.
  • Determining whether requested services meet coverage requirements based on clinical information and criteria.
  • Requesting additional clinical information from providers when documentation is incomplete.
  • Documenting clinical review decisions accurately and consistently.
  • Communicating review outcomes to providers, members, and facilities.
  • Escalating clinical denials or complex cases to the medical director or clinical leadership.
  • Supporting peer-to-peer review processes when clinically appropriate.
  • Maintaining compliance with regulatory turnaround times and accreditation standards.

Clinical judgment, criteria knowledge, and clear communication are essential — prior authorization RNs are the clinical reviewers who apply coverage criteria to support appropriate member access to care.

Prior Authorization Workflow

A Remote Prior Authorization RN may follow a structured workflow across each prior authorization review.

Depending on the employer's protocols, you may:

Receive PA Request Review Clinical Info Apply Criteria Determine Coverage Document Decision Communicate Outcome Escalate If Needed

Regulatory Compliance & Turnaround Times

Prior authorization directly affects member access to care, provider reimbursement, and regulatory compliance. Reviews must be conducted in accordance with evidence-based criteria, CMS requirements, NCQA/URAC accreditation standards, state regulations, and required turnaround times for urgent and non-urgent requests. Clinical denials and complex cases must be escalated to the medical director or clinical leadership following the employer's escalation protocol.

Required Qualifications

  • Active RN license in good standing in the state(s) required by the employer.
  • Previous clinical experience in prior authorization, utilization review, utilization management, case management, or acute care nursing.
  • Working knowledge of medical and pharmacy benefit criteria, prior authorization workflows, and payer clinical review.
  • Strong clinical review, criteria application, and documentation skills.
  • Comfortable with prior authorization platforms, EHR systems, and payer clinical review workflows.
  • Strong documentation and time-management skills.
  • Ability to recognize situations requiring clinical escalation.
  • Ability to work independently in a remote environment.
  • Ability to maintain confidentiality and HIPAA compliance.

A Typical Day

A typical day may begin by logging in, checking your prior authorization queue, and prioritizing requests based on urgency, turnaround deadlines, or review type.

You'll review clinical information submitted by providers and facilities against payer criteria (MCG, InterQual, payer-specific, or clinical guidelines). You'll determine whether requested services meet coverage requirements, document your clinical review decision, and communicate the review outcome to providers, members, and facilities.

When documentation is incomplete, you'll request additional clinical information from providers. For complex or denial reviews, you'll coordinate with medical directors and clinical leadership, and support peer-to-peer review processes when requested.

Throughout the day, you'll ensure compliance with turnaround times and regulatory requirements, maintain accurate documentation, and stay current on medical policies, pharmacy policies, criteria updates, and clinical guidelines.

The role combines clinical information review, criteria application, coverage determination support, documentation, provider and member communication, and appropriate clinical escalation.

Unlike utilization review nursing, which focuses broadly on medical necessity and level of care across service settings, prior authorization nursing focuses specifically on the front-end authorization process — you're the RN who reviews whether requested medical and pharmacy services meet coverage requirements before they're delivered.
