Remote Psychiatric Triage Nurse

Conduct psychiatric triage assessments, determine psychiatric acuity, and direct patients to the appropriate level of psychiatric care from home.

$34–$58 / hour 100% Remote Full-Time / Part-Time Multiple U.S. States

Job Overview

Virelia Health connects registered nurses with opportunities across virtual psychiatric triage, telehealth mental health assessment, and digital crisis navigation.

We are seeking qualified Psychiatric Triage Nurses for remote opportunities supporting telehealth psychiatric triage programs, virtual crisis lines, health systems, health plans, and digital behavioral health organizations.

A Remote Psychiatric Triage Nurse conducts psychiatric triage assessments, determines psychiatric acuity, and directs patients to the appropriate level of psychiatric care. Working within their state licensure and clinical scope of practice, the psychiatric triage nurse assesses mental health symptoms, evaluates suicide risk, applies psychiatric triage protocols, and directs patients to the right level of care — self-care, outpatient behavioral health, urgent psychiatric care, 988 crisis services, or emergency psychiatric evaluation.

This position is well-suited to a registered nurse with psychiatric nursing, behavioral health nursing, emergency psychiatric care, or telehealth triage experience who wants to practice in a virtual setting — providing meaningful psychiatric triage work with schedule flexibility and without the demands of a facility-based environment.

The specific psychiatric triage program, patient population, triage protocols, and scope of responsibilities will depend on the employer and assignment.

This opportunity may be suitable for professionals searching for remote psychiatric triage nurse jobs, work-from-home mental health triage nurse jobs, remote behavioral health triage nurse jobs, virtual psychiatric triage RN, or telehealth psychiatric triage nurse roles.

Scheduled Psychiatric Triage with Predictable Hours

Virtual psychiatric triage runs on scheduled patient contacts and shift blocks, giving your clinical day predictability and structure. Programs typically offer daytime, evening, overnight, or weekend coverage because psychiatric crises occur across all hours. Many programs offer flexible 8-, 10-, or 12-hour shifts, and some RNs work 4-day compressed weeks or part-time schedules. There are no facility floor assignments and no physical demands. We offer both full-time and part-time positions. Discuss your preferred schedule during your interview.

What You'll Do

As a Remote Psychiatric Triage Nurse, you may support patients across a range of virtual psychiatric triage encounters.

  • Conduct virtual psychiatric triage assessments via video or phone — assessing mental health symptoms, acuity, and urgency.
  • Apply psychiatric triage protocols to guide clinical decision-making.
  • Conduct suicide risk assessment — evaluating ideation, intent, plan, and means.
  • Conduct homicide risk assessment as clinically indicated.
  • Determine the appropriate level of psychiatric care — self-care, outpatient behavioral health, urgent psychiatric care, 988 crisis services, or emergency psychiatric evaluation.
  • Direct patients to the right level of psychiatric care based on clinical assessment and triage protocols.
  • Provide patient education on mental health conditions, symptom management, and coping skills.
  • Provide de-escalation and crisis intervention support.
  • Support safety planning — identifying warning signs, coping strategies, and support contacts.
  • Coordinate with 988 crisis services, mobile crisis teams, and emergency psychiatric services as clinically indicated.
  • Document each triage encounter accurately in the triage platform or EHR.
  • Escalate emergency psychiatric concerns to 988, 911, or emergency psychiatric services immediately.
  • Coordinate with behavioral health providers, psychiatric teams, and on-call clinical staff as needed.
  • Support patients with chronic mental health conditions — symptom monitoring and care plan reinforcement.
  • Provide reassurance and clear instructions to patients who are in distress.
  • Participate in psychiatric triage quality improvement, clinical review, and triage education.
  • Maintain current knowledge of psychiatric triage protocols, mental health guidelines, and licensure requirements.
  • Protect patient privacy and maintain HIPAA compliance across all virtual psychiatric triage interactions.

Psychiatric Triage Nursing Focus

Virtual psychiatric triage is a growing area of psychiatric nursing — driven by increased demand for mental health services, the 988 crisis line implementation, and the recognition that psychiatric triage nurses help patients in crisis get the right care at the right time. Psychiatric triage nurses extend mental health triage reach, improve access to crisis services, and support patients during some of their most vulnerable moments.

Depending on the program, psychiatric triage may focus on:

Telephonic psychiatric triage
Video psychiatric triage
After-hours psychiatric triage
Crisis triage
Suicide risk assessment
Behavioral health triage

The psychiatric triage nurse's role is to provide virtual psychiatric triage within their licensure and scope of practice — including psychiatric assessment, suicide risk assessment, application of triage protocols, acuity determination, and level-of-care direction. Clinical decisions, medication changes, and diagnostic decisions belong to the treating psychiatric providers; the psychiatric triage nurse supports the patient's triage encounter and coordinates with the behavioral health care team.

This structure keeps the role focused on psychiatric triage assessment, suicide risk evaluation, psychiatric acuity determination, level-of-care direction, documentation, and appropriate emergency escalation.

Suicide Risk Assessment & Crisis Navigation

Effective psychiatric triage nursing depends on strong suicide risk assessment and crisis navigation — the RN's clinical judgment and triage expertise determine how to help each patient in psychiatric distress access the right care.

Depending on the program and clinical focus, you may support patients by:

  • Conducting comprehensive psychiatric triage assessments — mental health symptoms, acuity, and urgency.
  • Conducting suicide risk assessment — evaluating ideation, intent, plan, and means.
  • Conducting homicide risk assessment as clinically indicated.
  • Identifying red-flag psychiatric symptoms and emergency-level concerns.
  • Applying psychiatric triage protocols accurately.
  • Determining the appropriate level of psychiatric care.
  • Providing de-escalation and crisis intervention support.
  • Supporting safety planning — identifying warning signs, coping strategies, and support contacts.
  • Coordinating with 988 crisis services, mobile crisis teams, and emergency psychiatric services.
  • Directing patients to 988, 911, or emergency psychiatric services immediately when clinically indicated.
  • Documenting each triage encounter accurately and consistently.
  • Coordinating with behavioral health providers, psychiatric teams, and on-call clinical staff.

Clinical judgment, empathy, and crisis intervention expertise are essential — psychiatric triage nursing requires clear communication, rapid clinical assessment, and disciplined protocol application during high-acuity moments.

Psychiatric Triage Workflow

A Remote Psychiatric Triage Nurse may follow a structured workflow across each virtual psychiatric triage encounter.

Depending on the employer's protocols, you may:

Answer Contact Identify Patient Assess Psychiatric Symptoms Conduct Suicide Risk Assessment Apply Triage Protocol Determine Level of Care Educate & Direct Document & Escalate If Needed

Emergency Psychiatric Escalation & Patient Safety

If a patient presents with emergency-level psychiatric symptoms — active suicidal ideation with intent or plan, active homicidal ideation, acute psychosis, or another psychiatric emergency — the triage RN must follow the employer's emergency escalation protocol immediately, including directing the patient to 988, 911, or emergency psychiatric services. Virtual psychiatric triage does not replace emergency psychiatric care.

Required Qualifications

  • Active RN license in good standing in the state(s) required by the employer.
  • Previous clinical experience in psychiatric nursing, behavioral health nursing, emergency psychiatric care, or telehealth triage.
  • Strong psychiatric assessment, crisis evaluation, and suicide risk assessment skills.
  • Comfortable with telehealth platforms, psychiatric triage protocols, and EHR systems.
  • Strong documentation and time-management skills.
  • Ability to recognize situations requiring emergency or clinical escalation.
  • Ability to work independently in a remote environment.
  • Ability to maintain confidentiality and HIPAA compliance.
  • Reliable internet suitable for real-time video and a private workspace suitable for confidential patient conversations.

A Typical Day

A typical day may begin by logging into the triage system, checking in with your shift lead, and reviewing any active case notes or follow-up items.

You'll answer psychiatric triage calls, chats, or video requests as they come in — conducting structured psychiatric assessments, evaluating suicide risk, and applying the appropriate triage protocols to determine each patient's level of care.

Between encounters, you'll document each triage interaction accurately, coordinate with behavioral health providers, 988 crisis services, and mobile crisis teams, and follow up with patients who may need additional support after triage.

Throughout the day, you'll identify emergency-level psychiatric concerns and follow the employer's emergency escalation protocol immediately — directing patients to 988, 911, or emergency psychiatric services as clinically indicated.

The role combines psychiatric assessment, suicide risk evaluation, triage protocol application, level-of-care direction, patient education, documentation, and appropriate emergency escalation.

Unlike medical virtual triage, psychiatric triage focuses specifically on mental health and behavioral health concerns — you're the RN who helps patients in psychiatric distress navigate their symptoms safely and access the right crisis resources.
