Remote Utilization Review RN

Review medical necessity and level of care for insurance programs — supporting utilization management with MCG and InterQual criteria from home.

$34–$52 / hour 100% Remote Full-Time / Part-Time Multiple U.S. States

Job Overview

Virelia Health connects registered nurses with opportunities across utilization management, medical necessity review, and payer clinical review.

We are seeking qualified Utilization Review RNs for remote opportunities supporting health plans, managed care organizations (MCOs), utilization management companies, and healthcare systems.

A Remote Utilization Review RN conducts medical necessity reviews, determines level of care, and supports utilization management for health plans and managed care organizations. Working within their state licensure and clinical scope of practice, the utilization review RN reviews clinical information against evidence-based criteria — MCG (Milliman Care Guidelines), InterQual, or payer-specific criteria — to determine medical necessity and appropriate level of care.

This position is well-suited to a registered nurse with utilization review, utilization management, case management, or acute care nursing experience who wants to practice in a virtual setting — providing meaningful clinical review work with schedule flexibility and without the demands of a facility-based environment.

The specific utilization review program, plan type, criteria set, and scope of responsibilities will depend on the employer and assignment.

This opportunity may be suitable for professionals searching for remote utilization review RN jobs, work-from-home UR nurse jobs, remote utilization management RN jobs, remote UM nurse, or remote utilization review nurse roles.

Structured Clinical Review with Predictable Hours

Utilization review runs on structured clinical review workflows — typically during weekday business hours, with some programs offering extended coverage. Many programs offer flexible 8- or 10-hour days, and some utilization review RNs work 4-day compressed weeks or part-time schedules. There are no facility floor assignments and no physical demands. We offer both full-time and part-time positions. Discuss your preferred schedule during your interview.

What You'll Do

As a Remote Utilization Review RN, you may support health plans and MCOs across the full utilization management lifecycle.

  • Review clinical information against evidence-based criteria — MCG, InterQual, or payer-specific criteria.
  • Determine medical necessity for requested services, admissions, and continued stays.
  • Determine appropriate level of care — inpatient, observation, outpatient, or alternative settings.
  • Conduct prospective review (pre-service), concurrent review (during service), and retrospective review (post-service) as assigned.
  • Apply utilization management criteria accurately and consistently.
  • Document clinical review decisions accurately in the utilization management platform.
  • Communicate with providers and facilities on clinical review outcomes.
  • Support peer-to-peer review processes when clinical denials are escalated.
  • Coordinate with medical directors on complex clinical reviews.
  • Support appeal reviews and second-level clinical reviews as assigned.
  • Identify opportunities for care optimization and alternative care settings.
  • Maintain compliance with regulatory requirements — CMS, NCQA, URAC, and state regulations.
  • Participate in utilization management quality improvement and clinical review.
  • Maintain current knowledge of clinical guidelines, criteria updates, and regulatory changes.
  • Protect member privacy and maintain HIPAA compliance across all utilization review activities.

Utilization Review Focus

Utilization review is a growing area of clinical practice — driven by payer investment in utilization management, health plan investment in medical necessity review, and the recognition that evidence-based clinical review helps ensure appropriate care delivery.

Depending on the program, utilization review may focus on:

Medical necessity review
Level of care determination
Prospective review
Concurrent review
Retrospective review
Criteria application (MCG/InterQual)

The utilization review RN's role is to provide clinical review within their licensure and scope of practice — including medical necessity review, level of care determination, criteria application, documentation, and appropriate escalation. Clinical decisions, ultimate medical necessity determinations, and clinical denial decisions belong to the medical director or clinical leadership; the utilization review RN supports the clinical review process and coordinates with clinical leadership.

This structure keeps the role focused on clinical review, criteria application, medical necessity determination, documentation, and appropriate escalation.

Criteria Application & Medical Necessity Determination

Effective utilization review depends on strong criteria application and medical necessity determination — the RN's clinical judgment and criteria knowledge determine the appropriate review outcome.

Depending on the program and clinical focus, you may support clinical review by:

  • Applying MCG (Milliman Care Guidelines) criteria accurately.
  • Applying InterQual criteria accurately.
  • Applying payer-specific or plan-specific criteria accurately.
  • Determining medical necessity based on clinical documentation and criteria.
  • Determining appropriate level of care based on clinical presentation and criteria.
  • Documenting clinical review decisions accurately and consistently.
  • Communicating clinical review outcomes to providers and facilities.
  • Escalating clinical denials or complex cases to the medical director or clinical leadership.
  • Supporting peer-to-peer review processes when clinically appropriate.
  • Maintaining compliance with regulatory and accreditation requirements.

Clinical judgment, criteria knowledge, and clear communication are essential — utilization review RNs are the clinical reviewers who apply evidence-based criteria to support appropriate care delivery.

Utilization Review Workflow

A Remote Utilization Review RN may follow a structured workflow across each clinical review.

Depending on the employer's protocols, you may:

Regulatory Compliance & Clinical Escalation

Utilization review directly affects member access to care, provider reimbursement, and regulatory compliance. Reviews must be conducted in accordance with evidence-based criteria, CMS requirements, NCQA/URAC accreditation standards, and applicable state regulations. Clinical denials and complex cases must be escalated to the medical director or clinical leadership following the employer's escalation protocol.

Required Qualifications

  • Active RN license in good standing in the state(s) required by the employer.
  • Previous clinical experience in utilization review, utilization management, case management, or acute care nursing.
  • Working knowledge of MCG and InterQual criteria.
  • Strong clinical review, medical necessity determination, and clinical documentation skills.
  • Comfortable with utilization management platforms, EHR systems, and payer clinical review workflows.
  • Strong documentation and time-management skills.
  • Ability to recognize situations requiring clinical escalation.

A Typical Day

A typical day may begin by logging in, checking your utilization review queue, and prioritizing cases based on urgency, admission date, or review type.

You'll review clinical information — medical records, provider notes, and clinical documentation — against the appropriate criteria (MCG, InterQual, or payer-specific). You'll determine medical necessity and level of care, document your clinical review decision accurately, and communicate the review outcome to providers and facilities.

Between reviews, you'll coordinate with medical directors on complex clinical decisions, support peer-to-peer review processes, and follow up on escalated cases.

Throughout the day, you'll ensure compliance with regulatory requirements, maintain accurate documentation, and stay current on criteria updates and clinical guidelines.

The role combines medical necessity review, criteria application, level of care determination, clinical documentation, provider communication, and appropriate clinical escalation.

Unlike direct patient care roles, utilization review nursing focuses on clinical review — you're the RN who applies evidence-based criteria to support appropriate care delivery.

Remote Work Environment

This position is designed for remote utilization review delivery.

Depending on the employer, you may work with:
