Remote Virtual Discharge Nurse

Support post-discharge virtual follow-up — RN-led discharge support, medication reconciliation support, and care transition nursing from home.

$32–$55 / hour 100% Remote Full-Time / Part-Time Multiple U.S. States

Job Overview

Virelia Health connects registered nurses with opportunities across virtual discharge support, post-discharge follow-up, and care transition nursing.

We are seeking qualified Virtual Discharge Nurses for remote opportunities supporting virtual discharge programs, health systems, health plans, accountable care organizations (ACOs), and digital care organizations.

A Remote Virtual Discharge Nurse supports post-discharge virtual follow-up — RN-led discharge support, medication reconciliation support, and care transition nursing. Working within their state licensure and clinical scope of practice, the virtual discharge nurse supports members through the critical post-discharge window — reviewing discharge instructions, supporting medication reconciliation, screening for red flags, and coordinating follow-up care.

This position is well-suited to a registered nurse with acute care, discharge planning, transitions of care, or virtual care nursing experience who wants to practice in a virtual setting — providing meaningful clinical discharge support with schedule flexibility and without the demands of a facility-based environment.

The specific virtual discharge program, member population, clinical model, and scope of responsibilities will depend on the employer and assignment.

This opportunity may be suitable for professionals searching for remote virtual discharge nurse jobs, work-from-home discharge nurse jobs, remote discharge RN jobs, virtual post-discharge RN, or remote transitions of care RN roles.

Scheduled Virtual Discharge Support with Predictable Hours

Virtual discharge nursing runs on scheduled post-discharge outreach blocks, giving your clinical day predictability and structure. Programs typically offer daytime, evening, or weekend coverage to accommodate member availability and post-discharge follow-up timelines. Many programs offer flexible 8- or 10-hour shifts, and some RNs work 4-day compressed weeks or part-time schedules. There are no facility floor assignments and no physical demands. We offer both full-time and part-time positions. Discuss your preferred schedule during your interview.

What You'll Do

As a Remote Virtual Discharge Nurse, you may support members across the full post-discharge virtual follow-up.

  • Complete post-discharge outreach calls within the program's required timeframe — typically within 24–72 hours of discharge.
  • Review discharge instructions with the member and confirm understanding.
  • Support medication reconciliation — reviewing discharge medications, identifying discrepancies, and connecting members with resources.
  • Confirm follow-up appointments are scheduled and support members in getting to them.
  • Assess member needs post-discharge — clinical, functional, and social.
  • Screen for red flags and warning signs that may indicate deterioration.
  • Educate members on warning signs and when to seek care.
  • Support members in understanding their discharge diagnosis and care plan.
  • Coordinate with home health, DME, and community resources as needed.
  • Identify social barriers to recovery — transportation, food, housing — and connect members with resources.
  • Document all post-discharge contacts accurately in the transitions platform.
  • Coordinate with the member's care team — PCP, specialists, home health, and care manager.
  • Escalate clinical concerns, medication issues, or social barriers to the appropriate team.
  • Follow emergency escalation protocols for members reporting acute symptoms.
  • Participate in interdisciplinary care team meetings and transition case reviews.
  • Protect member privacy and maintain HIPAA compliance across all virtual discharge activities.

Virtual Discharge Nursing Focus

Virtual discharge nursing is a growing area of clinical practice — driven by increased demand for post-discharge support, payer investment in readmission reduction, and health system investment in digital care delivery. Virtual discharge nurses support members through the critical post-discharge window, when the risk of readmission, medication errors, and missed follow-up is highest.

Depending on the program, virtual discharge nursing may focus on:

Post-discharge follow-up
Medication reconciliation support
Follow-up appointment support
Red flag screening
Social needs support
Readmission prevention

The virtual discharge nurse's role is to provide virtual discharge nursing within their licensure and scope of practice — including post-discharge assessment, medication reconciliation support, red flag screening, follow-up coordination, documentation, and care coordination. Clinical decisions, medication changes, and diagnostic decisions belong to the treating providers; the virtual discharge nurse supports the member's post-discharge recovery and coordinates with the care team.

This structure keeps the role focused on post-discharge assessment, medication support, follow-up coordination, documentation, and appropriate clinical escalation.

Post-Discharge Support & Readmission Prevention

Effective virtual discharge nursing depends on strong post-discharge support and readmission prevention — the RN's clinical judgment and follow-up rigor determine how to help each member recover safely at home.

Depending on the program and clinical focus, you may support members by:

  • Reviewing discharge instructions and confirming member understanding.
  • Supporting medication reconciliation — identifying discrepancies and connecting members with resources.
  • Confirming follow-up appointments are scheduled and supporting members in getting there.
  • Screening for warning signs — worsening symptoms, medication issues, and functional decline.
  • Educating members on warning signs and when to seek care.
  • Identifying social barriers — transportation, food, housing, and caregiver support — and connecting members with resources.
  • Coordinating with home health, DME, and community resources.
  • Documenting all contacts and escalating concerns promptly.
  • Following up consistently through the 7-day and 30-day windows.

Clinical judgment, member empathy, and rigorous follow-up are essential — virtual discharge nurses are the safety net that helps members recover successfully at home.

Virtual Discharge Workflow

A Remote Virtual Discharge Nurse may follow a structured workflow across each post-discharge encounter.

Depending on the employer's protocols, you may:

Receive Discharge Notification Outreach Member Review Discharge Instructions Support Med Reconciliation Confirm Follow-Up Document & Follow Up Escalate If Needed

Clinical Escalation & Member Safety

If a member reports acute clinical symptoms, medication concerns, safety concerns, or other urgent needs during any virtual interaction, the RN must follow the employer's clinical escalation protocol immediately — including directing the member to call 911, contact their provider, or seek emergency care as appropriate. Virtual discharge support does not replace emergency care.

Required Qualifications

  • Active RN license in good standing in the state(s) required by the employer.
  • Previous clinical experience in acute care, discharge planning, transitions of care, case management, or virtual care nursing.
  • Strong clinical assessment, discharge support, and care transition skills.
  • Comfortable with post-discharge virtual follow-up workflows, telehealth platforms, and EHR systems.
  • Strong documentation and time-management skills.
  • Ability to recognize situations requiring clinical escalation.
  • Ability to work independently in a remote environment.
  • Ability to maintain confidentiality and HIPAA compliance.
  • Reliable internet suitable for real-time video and a private workspace suitable for confidential member conversations.

A Typical Day

A typical day may begin by reviewing your post-discharge follow-up list — checking which members were recently discharged and are due for outreach within the required timeframe.

You'll call members who have recently been discharged — reviewing discharge instructions, supporting medication reconciliation, confirming follow-up appointments, and screening for red flags.

Between calls, you'll coordinate with home health, DME, and community resources, document each contact accurately, and follow up on members who haven't been reached.

Throughout the day, you'll escalate members with clinical concerns, medication issues, or social barriers to the appropriate team, and follow up through the 7-day and 30-day windows.

The role combines post-discharge outreach, medication reconciliation support, follow-up coordination, red flag screening, documentation, and appropriate clinical escalation.

Unlike transitions of care coordination, which may be non-clinical, virtual discharge nursing is RN-led — you're the clinical professional who assesses members, supports medication reconciliation, screens for warning signs, and coordinates follow-up during the critical post-discharge window.
