Remote Telehealth Case Manager RN

Support complex case management — RN-led virtual case management, complex case coordination, and resource management from home.

$34–$58 / hour 100% Remote Full-Time / Part-Time Multiple U.S. States

Job Overview

Virelia Health connects registered nurses with opportunities across virtual case management, telehealth case management, and digital care programs.

We are seeking qualified Telehealth Case Manager RNs for remote opportunities supporting virtual case management programs, health systems, health plans, accountable care organizations (ACOs), and digital care organizations.

A Remote Telehealth Case Manager RN supports complex case management — RN-led virtual case management, complex case coordination, and resource management. Working within their state licensure and clinical scope of practice, the telehealth case manager RN supports members with complex clinical and social needs through virtual assessment, case management planning, resource coordination, and ongoing member support.

This position is well-suited to a registered nurse with case management, complex care management, utilization review, or virtual care nursing experience who wants to practice in a virtual setting — providing meaningful clinical case management work with schedule flexibility and without the demands of a facility-based environment.

The specific telehealth case management program, member population, clinical model, and scope of responsibilities will depend on the employer and assignment.

This opportunity may be suitable for professionals searching for remote telehealth case manager RN jobs, work-from-home case manager RN jobs, remote RN case manager jobs, virtual case manager RN, or remote complex case management RN roles.

Scheduled Virtual Case Management with Predictable Hours

Virtual case management runs on scheduled member outreach and coordination blocks, giving your clinical day predictability and structure. Programs typically offer daytime, evening, or weekend coverage to accommodate member availability. Many programs offer flexible 8- or 10-hour shifts, and some RNs work 4-day compressed weeks or part-time schedules. There are no facility floor assignments and no physical demands. We offer both full-time and part-time positions. Discuss your preferred schedule during your interview.

What You'll Do

As a Remote Telehealth Case Manager RN, you may support members across the full virtual case management lifecycle.

  • Conduct comprehensive virtual member assessments — clinical, functional, psychosocial, and social needs.
  • Develop individualized virtual case management plans in collaboration with members, caregivers, and the interdisciplinary team.
  • Coordinate complex care across multiple providers, specialists, and community resources through virtual channels.
  • Support members with complex clinical and social needs — chronic conditions, behavioral health, and social determinants.
  • Support utilization management — reviewing level of care and care setting appropriateness within scope.
  • Support care transitions — post-discharge, post-ED, and specialist referrals.
  • Facilitate interdisciplinary case coordination — virtual case conferences and care team huddles.
  • Coordinate with virtual care teams — providers, care managers, behavioral health, and social services.
  • Reinforce provider care plans and clinical instructions.
  • Coordinate resources — home health, DME, transportation, and community resources.
  • Document all member contacts accurately in the case management platform.
  • Identify members with rising risk or unmet needs and escalate to the appropriate clinical team.
  • Support members in crisis — following crisis escalation protocols.
  • Participate in virtual case management quality improvement, clinical review, and education.
  • Maintain current knowledge of case management best practices, utilization management, and licensure requirements.
  • Protect member privacy and maintain HIPAA compliance across all virtual case management interactions.

Telehealth Case Management Focus

Virtual case management is a growing area of clinical practice — driven by increased demand for complex care coordination, payer investment in case management, and health system investment in digital care delivery. Telehealth case manager RNs extend clinical reach, improve complex care coordination, and support members with complex needs.

Depending on the program, telehealth case management may focus on:

Complex case management
High-risk member support
Utilization management support
Care transitions
Resource management
Interdisciplinary coordination

The telehealth case manager RN's role is to provide virtual case management within their licensure and scope of practice — including assessment, case management planning, complex care coordination, resource management, documentation, and care navigation. Clinical decisions, medication changes, and diagnostic decisions belong to the treating providers; the telehealth case manager RN supports the member's complex care and coordinates with the interdisciplinary care team.

This structure keeps the role focused on virtual member assessment, complex case management planning, interdisciplinary coordination, resource management, and appropriate clinical escalation.

Complex Case Coordination & Resource Management

Effective virtual case management depends on strong complex case coordination and resource management — the RN's clinical judgment and coordination skills determine how to help each member navigate complex care needs.

Depending on the program and clinical focus, you may support members by:

  • Conducting comprehensive virtual member assessments — clinical, functional, psychosocial, and social needs.
  • Developing individualized virtual case management plans with members, caregivers, and the interdisciplinary team.
  • Coordinating complex care across multiple providers, specialists, and community resources.
  • Supporting utilization management — reviewing level of care and care setting appropriateness within scope.
  • Supporting care transitions — post-discharge, post-ED, and specialist referrals.
  • Facilitating interdisciplinary case coordination — virtual case conferences and care team huddles.
  • Coordinating resources — home health, DME, transportation, and community resources.
  • Documenting each member contact accurately and consistently.
  • Escalating concerning clinical findings to the appropriate clinical team.
  • Following crisis escalation protocols when members report urgent clinical symptoms.

Clinical judgment, coordination skills, and clear communication are essential — virtual case management requires strong interdisciplinary collaboration, resource management, and consistent member engagement across virtual channels.

Virtual Case Management Workflow

A Remote Telehealth Case Manager RN may follow a structured workflow across each virtual case management encounter.

Depending on the employer's protocols, you may:

Review Member Panel Virtual Outreach Comprehensive Assessment Develop Case Plan Coordinate Resources Document Encounter Escalate If Needed

Clinical Escalation & Member Safety

If a member reports acute clinical symptoms, urgent safety concerns, or other urgent needs during any virtual interaction, the RN must follow the employer's clinical escalation protocol immediately — including directing the member to call 911, contact their provider, or seek emergency care as appropriate. Virtual case management does not replace emergency care.

Required Qualifications

  • Active RN license in good standing in the state(s) required by the employer.
  • Previous clinical experience in case management, complex care management, utilization review, or virtual care nursing.
  • Strong complex case management, clinical assessment, and resource coordination skills.
  • Comfortable with case management platforms, telehealth platforms, and EHR systems.
  • Strong documentation and time-management skills.
  • Ability to recognize situations requiring clinical escalation.
  • Ability to work independently in a remote environment.
  • Ability to maintain confidentiality and HIPAA compliance.
  • Reliable internet suitable for real-time video and a private workspace suitable for confidential member conversations.

A Typical Day

A typical day may begin by reviewing your virtual case management panel — checking member names, recent clinical notes, high-risk flags, and any case management needs that require follow-up.

You'll conduct virtual member outreach by phone or video — completing comprehensive assessments, developing case management plans, and coordinating with members and their care teams on care plan development and reinforcement.

Between member encounters, you'll facilitate interdisciplinary case coordination, coordinate complex care across providers and specialists, coordinate resources, and document each member contact accurately.

Throughout the day, you'll support members with complex needs, support utilization management within scope, escalate concerning clinical findings to the appropriate clinical team, and follow up with members who need extra support.

The role combines virtual member assessment, complex case management planning, interdisciplinary coordination, resource management, documentation, and appropriate clinical escalation.

Unlike telehealth care coordination, which focuses on care planning and coordination across the care team, telehealth case management focuses specifically on complex, high-risk members — you're the RN who manages the most complex cases and coordinates complex care across the healthcare system.
