Remote Remote Patient Monitoring RN

Support RPM programs with RN-level clinical oversight — reviewing monitoring data, patient outreach, and clinical escalation from home.

$32–$52 / hour 100% Remote Full-Time / Part-Time Multiple U.S. States

Job Overview

Virelia Health connects registered nurses with opportunities across remote patient monitoring, RPM clinical oversight, and digital chronic care programs.

We are seeking qualified Remote Patient Monitoring RNs for remote opportunities supporting RPM companies, health systems, health plans, accountable care organizations (ACOs), and digital chronic care organizations.

A Remote Remote Patient Monitoring RN supports RPM programs — clinical monitoring oversight, RPM data review, and RN-led patient escalation. Working within their state licensure and clinical scope of practice, the RPM RN reviews patient monitoring data, conducts patient outreach, reinforces care plans, and escalates clinical concerns to the appropriate provider.

This position is well-suited to a registered nurse with chronic care management, ambulatory care, telehealth, or remote patient monitoring experience who wants to practice in a virtual setting — providing meaningful clinical monitoring work with schedule flexibility and without the demands of a facility-based environment.

The specific RPM program, patient population, clinical model, and scope of responsibilities will depend on the employer and assignment.

This opportunity may be suitable for professionals searching for remote remote patient monitoring RN jobs, work-from-home RPM RN jobs, remote RPM nursing jobs, virtual RPM nurse, or remote patient monitoring nurse roles.

Scheduled RPM Clinical Oversight with Predictable Hours

Remote patient monitoring runs on scheduled monitoring and outreach blocks, giving your clinical day predictability and structure. Programs typically offer daytime or evening coverage to accommodate patient needs. Many programs offer flexible 8- or 10-hour shifts, and some RPM RNs work 4-day compressed weeks or part-time schedules. There are no facility floor assignments and no physical demands. We offer both full-time and part-time positions. Discuss your preferred schedule during your interview.

What You'll Do

As a Remote Remote Patient Monitoring RN, you may support patients across the full RPM monitoring lifecycle.

  • Review RPM monitoring data — blood pressure, blood glucose, weight, pulse oximetry, and other metrics.
  • Identify patients with concerning trends or out-of-range readings.
  • Conduct virtual patient outreach by phone or video — assessing symptoms and clinical status.
  • Provide clinical monitoring oversight for the RPM program — supporting RN-level clinical review.
  • Reinforce care plans and provider instructions based on monitoring data.
  • Provide patient education on condition management, monitoring, and self-care.
  • Support patients with medication understanding and adherence.
  • Support patients with chronic conditions — diabetes, hypertension, COPD, heart failure, and asthma.
  • Escalate concerning clinical findings to the appropriate provider or clinical team.
  • Coordinate with providers, care managers, and the interdisciplinary care team.
  • Document all patient contacts accurately in the RPM platform and EHR.
  • Support patients with device setup and troubleshooting coordination.
  • Support RPM program quality improvement and clinical review.
  • Participate in interdisciplinary care team meetings and clinical case reviews.
  • Maintain current knowledge of RPM best practices, clinical guidelines, and licensure requirements.
  • Protect patient privacy and maintain HIPAA compliance across all RPM interactions.

Remote Patient Monitoring Focus

Remote patient monitoring is a growing area of clinical practice — driven by increased demand for chronic condition monitoring, payer coverage for RPM services, and health system investment in digital chronic care delivery. RPM RNs extend clinical reach, improve chronic condition outcomes, and support patients between visits.

Depending on the program, RPM nursing may focus on:

Blood pressure monitoring
Blood glucose monitoring
Weight monitoring
Pulse oximetry monitoring
Heart failure monitoring
COPD monitoring

The RPM RN's role is to provide RN-level clinical monitoring within their licensure and scope of practice — including data review, patient outreach, clinical assessment, care plan reinforcement, documentation, and appropriate clinical escalation. Clinical decisions, medication changes, and diagnostic decisions belong to the treating providers; the RPM RN supports the patient's chronic care and coordinates with the care team.

This structure keeps the role focused on RPM data review, patient outreach, clinical assessment, care plan reinforcement, documentation, and appropriate clinical escalation.

RPM Data Review & Clinical Escalation

Effective RPM nursing depends on strong data review and clinical escalation — the RN's clinical judgment and monitoring rigor determine how to help each patient stay on track between visits.

Depending on the program and clinical focus, you may support patients by:

  • Reviewing RPM monitoring data — blood pressure, blood glucose, weight, pulse oximetry, and other metrics.
  • Identifying patients with concerning trends or out-of-range readings.
  • Conducting virtual patient outreach by phone or video — assessing symptoms and clinical status.
  • Reinforcing care plans and provider instructions based on monitoring data.
  • Providing patient education on condition management, monitoring, and self-care.
  • Supporting patients with medication understanding and adherence.
  • Escalating concerning clinical findings to the appropriate provider or clinical team.
  • Coordinating with providers, care managers, and the interdisciplinary care team.
  • Documenting all patient contacts accurately and consistently.
  • Following crisis escalation protocols when patients report urgent clinical symptoms.

Clinical judgment, data analysis skills, and clear communication are essential — RPM RNs are the clinical reviewers who help patients manage their conditions successfully between visits.

RPM Workflow

A Remote Remote Patient Monitoring RN may follow a structured workflow across each RPM monitoring cycle.

Depending on the employer's protocols, you may:

Review RPM Data Identify Concerns Patient Outreach Assess Patient Reinforce Care Plan Document Encounter Escalate If Needed

Clinical Escalation & Patient Safety

If a patient reports acute clinical symptoms, urgent safety concerns, or other urgent needs during any RPM interaction, the RN must follow the employer's clinical escalation protocol immediately — including directing the patient to call 911, contact their provider, or seek emergency care as appropriate. RPM does not replace emergency care.

Required Qualifications

  • Active RN license in good standing in the state(s) required by the employer.
  • Previous clinical experience in chronic care management, ambulatory care, telehealth, or remote patient monitoring.
  • Strong clinical monitoring, data review, patient outreach, and clinical escalation skills.
  • Comfortable with RPM platforms, telehealth platforms, and EHR systems.
  • Strong documentation and time-management skills.
  • Ability to recognize situations requiring clinical escalation.
  • Ability to work independently in a remote environment.
  • Ability to maintain confidentiality and HIPAA compliance.
  • Reliable internet suitable for real-time video and a private workspace suitable for confidential patient conversations.

A Typical Day

A typical day may begin by reviewing your RPM monitoring dashboard — checking which patients have recent readings, which have out-of-range values, and which need outreach.

You'll review monitoring data for your assigned patients, identify concerning trends, and conduct virtual patient outreach — assessing symptoms, reinforcing care plans, and providing education based on monitoring data.

Between encounters, you'll document each patient contact accurately, coordinate with providers and care managers, and follow up on patients who need extra support.

Throughout the day, you'll escalate concerning clinical findings to the appropriate provider, support patients with device setup questions, and participate in care team coordination.

The role combines RPM data review, patient outreach, clinical assessment, care plan reinforcement, documentation, and appropriate clinical escalation.

Unlike LPN-scope RPM support or non-clinical monitoring specialist roles, the RPM RN provides RN-level clinical oversight — you're the clinical professional who interprets monitoring data, assesses patients, and makes clinical escalation decisions.

Remote Work Environment

This position is designed for remote RPM nursing delivery.

Depending on the employer, you may work with:

RPM platforms
Telehealth platforms
EHR / EMR
Secure messaging systems
Patient portals
Telehealth telephony

Candidates should have a stable internet connection suitable for real-time video, a private workspace suitable for confidential patient conversations, and comfort with multiple RPM and clinical systems.

Licensure & State Eligibility

Remote remote patient monitoring RN roles are currently in highest demand in states with strong RPM adoption, payer coverage for RPM services, and broad telework-friendly regulations:

Top-demand states: California, Texas, Florida, New York, Pennsylvania, Ohio, Illinois, Michigan, Georgia, North Carolina, Arizona, Colorado.

Strong secondary markets: Connecticut, Indiana, Maryland, Massachusetts, Minnesota, Missouri, New Jersey, Tennessee, Virginia, Washington, Wisconsin.

Always confirm the specific licensure requirements with the employer before applying.
