Remote LVN Case Management

Follow assigned patient cases over time — coordinating services, monitoring care plans, and supporting continuity of care from home.

$25–$37 / hour 100% Remote Full-Time / Part-Time Multiple U.S. States

Job Overview

Virelia Health connects qualified healthcare professionals with opportunities across nursing, clinical care, telehealth, and healthcare operations.

We are seeking qualified LVNs/LPNs for remote case-management opportunities supporting patients who may require ongoing healthcare coordination, follow-up, and assistance navigating their treatment and available services.

A Remote LVN Case Management role is centered on following assigned patient cases over time rather than handling a single interaction or isolated administrative request. The LVN may monitor care-plan activities, conduct patient outreach, identify barriers to care, coordinate services, document progress, and communicate relevant information to the appropriate members of the healthcare team.

This opportunity may be suitable for an LVN who enjoys building continuity with patients and helping keep healthcare plans organized.

Because remote nursing requirements vary by employer and patient population, specific responsibilities and scope of practice will depend on the individual assignment.

This opportunity may be suitable for professionals searching for remote LVN case management jobs, remote LVN case manager, remote LVN care management, remote LPN case management, work-from-home LVN case manager, or remote vocational nurse case management roles.

Flexible Scheduling Available

We understand that everyone's situation is different. That's why we offer both full-time and part-time positions with flexible hours. Whether you're looking for a standard 40-hour week, need to work around family commitments, want to supplement another job, or simply prefer a lighter schedule, we can accommodate your needs. Discuss your preferred schedule during your interview.

What You'll Do

As a Remote LVN Case Management professional, you may support patients throughout an ongoing episode of care.

  • Manage and monitor assigned patient cases according to established workflows.
  • Conduct telephone or approved digital outreach to patients.
  • Follow up with patients regarding ongoing healthcare needs and recommended services.
  • Review available patient information to understand current case status and outstanding needs.
  • Monitor progress toward established care-plan activities.
  • Help patients understand their next steps within the healthcare process.
  • Coordinate healthcare services with appropriate providers and organizations.
  • Follow up on referrals, appointments, tests, and other outstanding healthcare activities.
  • Identify barriers that may interfere with a patient's ability to access or complete recommended services.
  • Communicate relevant case information to appropriate healthcare professionals.
  • Document patient contacts, case activities, follow-up actions, and outcomes accurately.
  • Maintain organized case records within the applicable electronic healthcare system.
  • Escalate clinical concerns or situations outside the LVN/LPN scope of practice according to organizational procedures.
  • Participate in case-management meetings and team communication when required.
  • Follow established policies for privacy, documentation, communication, and patient safety.
  • Support continuity of care by maintaining timely and consistent follow-up.
  • Assist with identifying unresolved patient needs that require additional attention.

Case Management Focus

The main difference between case management and a single patient-service interaction is continuity.

A Remote LVN Case Management professional may remain involved with an assigned patient or group of patients while healthcare needs are being addressed.

The work may involve a cycle such as:

Assigned Case Assessment Info Care-Plan Review Patient Outreach Service Coordination Follow-Up Documentation Escalation When Needed

For example, a patient may have several healthcare services that need to be completed over a period of time. The LVN may help track those activities, communicate with the patient, coordinate with appropriate healthcare contacts, and document progress.

The goal is to help prevent important follow-up activities from being overlooked.

Patient Support & Follow-Up

Patient communication is an important part of this position.

Depending on the assignment, you may:

  • Contact patients about outstanding healthcare needs.
  • Explain routine care-management information using approved resources.

  • Identify difficulties accessing recommended care.
  • Follow up after healthcare encounters.
  • Communicate patient concerns to the appropriate team member.
  • Help connect patients with available healthcare resources.
  • Maintain consistent communication throughout the assigned case.

Clinical Escalation

Clinical questions requiring professional judgment beyond the LVN/LPN's permitted role should be escalated according to the employer's procedures.

Required Qualifications

  • Current LPN or LVN license applicable to the specific position.
  • Nursing education appropriate for LPN/LVN practice.
  • Healthcare or nursing experience.

You may contact patients, review outstanding case activities, communicate with healthcare providers, coordinate services, update electronic records, and document progress.

Some cases may require repeated communication over time. You may need to determine which activities have been completed, which remain outstanding, and which issues should be escalated to another member of the healthcare team.

Depending on the employer, Remote LVN Case Management professionals may work with:

Electronic health records
Case-management platforms
Patient portals

Licensure & State Eligibility

Remote nursing work can involve state-specific licensing requirements because healthcare services may be provided to patients located in different states.

Why Consider Remote LVN Case Management?

Remote case management can provide LVNs with an opportunity to use their nursing background in a patient-focused role that emphasizes continuity, follow-up, coordination, and healthcare navigation.

Instead of focusing exclusively on direct clinical encounters, case-management work can involve supporting patients over an extended period and helping coordinate the different steps involved in their healthcare.

This can be particularly relevant for LVNs with experience in clinical environments who also enjoy communication, organization, patient support, and coordinating services.
