Remote Clinical Pharmacist

Provide virtual pharmacy care — medication therapy management (MTM), medication reconciliation, and clinical pharmacy consultation — from home.

$45–$72 / hour 100% Remote Full-Time / Part-Time Multiple U.S. States

Job Overview

Virelia Health connects Clinical Pharmacists with opportunities across virtual pharmacy care, medication therapy management (MTM), and remote medication reconciliation.

We are seeking qualified Clinical Pharmacists (PharmD) for remote opportunities supporting telehealth pharmacy companies, health systems, managed care organizations, ambulatory care clinics, and healthcare organizations.

A Remote Clinical Pharmacist provides virtual medication therapy management, medication reconciliation, and clinical pharmacy consultation for patients. Working within their pharmacist scope, the clinical pharmacist conducts comprehensive medication reviews, supports chronic disease medication management, provides medication counseling, and supports medication safety through secure telehealth platforms.

This position is well-suited to an experienced clinical pharmacist with MTM knowledge, medication reconciliation experience, and clinical pharmacy expertise who wants to work in a virtual setting — providing meaningful clinical pharmacy care with schedule flexibility and without the demands of a facility-based environment.

The specific clinical pharmacy specialty, patient population, and scope of responsibilities will depend on the employer and assignment.

This opportunity may be suitable for professionals searching for remote clinical pharmacist jobs, work-from-home pharmacist jobs, virtual pharmacist jobs, telehealth pharmacy jobs, remote MTM pharmacist jobs, or virtual clinical pharmacist roles.

Structured Virtual Pharmacy Care with Predictable Hours

Virtual pharmacy care runs on scheduled patient consultation sessions and pharmacy documentation workflows, giving your workday predictability and structure. Programs typically offer daytime weekday coverage, with some programs supporting early-morning or early-evening patient sessions. Many programs offer flexible 8- or 10-hour days, and some pharmacists work 4-day compressed weeks or part-time schedules. There are no facility floor assignments and no physical demands. We offer both full-time and part-time positions. Discuss your preferred schedule during your interview.

  • Provide medication therapy management (MTM) — delivering MTM services based on national standards.

  • Coordinate with providers, care teams, and pharmacy teams as needed.
  • Support clinical pharmacy quality — maintaining clinical pharmacy quality and effectiveness.
  • Support patient engagement — supporting patient engagement and medication self-management.
  • Participate in clinical pharmacy quality improvement and program improvement.
  • Maintain current knowledge of pharmacotherapy, medication safety, and evidence-based clinical pharmacy guidelines.
  • Protect patient data and maintain HIPAA compliance across all virtual clinical pharmacy work.

Virtual Clinical Pharmacist Focus

Virtual clinical pharmacy is a growing area of healthcare — driven by increased demand for pharmacist services, expanded telehealth coverage for pharmacy care, and the recognition that clinical pharmacists improve patient medication outcomes through medication therapy management and medication safety. Virtual clinical pharmacists extend pharmacy care reach, support patients in their communities, and improve medication outcomes across organizations.

Depending on the program, virtual clinical pharmacy may focus on:

Medication therapy management (MTM)
Medication reconciliation
Chronic disease medication management
Anticoagulation management
Medication safety
Pharmacy compliance

The clinical pharmacist's role is to provide clinical pharmacy care within their pharmacist scope — including comprehensive medication review, medication therapy management, medication reconciliation, chronic disease medication management, medication safety, documentation, and pharmacy care quality. Prescribing decisions, diagnosis, and clinical management recommendations remain with the supervising provider; the clinical pharmacist provides clinical pharmacy consultation within their pharmacist scope and coordinates with providers, care teams, and pharmacy teams.

This structure keeps the role focused on medication therapy management, medication reconciliation, chronic disease medication support, documentation, and medication safety.

Medication Reconciliation & Chronic Disease Medication Management Support

Effective virtual clinical pharmacy depends on strong comprehensive medication review skills and medication safety expertise — the clinical pharmacist's clinical skills and patient-centered counseling approach determine how to help patients achieve medication outcomes through effective clinical pharmacy care.

Depending on the program and clinical focus, you may support virtual clinical pharmacy by:

  • Conducting comprehensive medication reviews — CMRs and re-reviews.
  • Providing medication therapy management — MTM services.
  • Conducting medication reconciliation — across providers, pharmacies, and transitions.
  • Supporting chronic disease medication management — diabetes, hypertension, dyslipidemia, and other conditions.
  • Supporting anticoagulation management — anticoagulation therapy and monitoring.
  • Supporting polypharmacy review — polypharmacy concerns and medication risks.
  • Supporting medication adherence — adherence and barrier identification.
  • Supporting medication safety — drug interactions, adverse effects, medication safety concerns.
  • Supporting medication counseling — patient medication counseling and education.
  • Supporting prescriber consultation — medication optimization and therapeutic alternatives.
  • Supporting prior authorization — prior authorization and formulary management.
  • Supporting clinical pharmacy quality improvement and program improvement.

Clinical expertise, patient-centered counseling skills, and clear communication are essential — clinical pharmacists are the medication experts who help patients achieve medication outcomes through effective clinical pharmacy care.

A Remote Clinical Pharmacist may follow a structured workflow across each virtual clinical pharmacy encounter.

Depending on the employer's protocols, you may:

Review Patient Referral Conduct Comprehensive Med Review Reconcile Medications Identify Med-Related Problems Provide Recommendations Counsel Patient & Prescriber Document & Escalate If Needed

Clinical Documentation & HIPAA Compliance

Virtual clinical pharmacy directly affects patient medication outcomes, patient safety, and clinical documentation. All clinical pharmacy care must comply with pharmacist scope of practice, state licensure requirements, evidence-based pharmacotherapy guidelines, and organizational clinical policy. Clinical concerns requiring clinical leadership input, patient safety concerns, or medication issues affecting patient outcomes must be escalated following the employer's escalation protocol. Clinical pharmacists must maintain HIPAA compliance and protect patient data across all virtual clinical pharmacy work.

Required Qualifications

  • Doctor of Pharmacy (PharmD) degree and active pharmacist state licensure required.
  • Previous experience as a clinical pharmacist in ambulatory care, hospital, managed care, or telehealth settings.
  • Strong medication therapy management (MTM) knowledge, medication reconciliation experience, and clinical pharmacy consultation skills.

A Typical Day

A typical day may begin by reviewing patient referrals, scheduled MTM consultations, and pending medication reconciliation tasks — checking virtual clinical pharmacy priorities and patient caseload.

You'll conduct comprehensive medication reviews — completing CMRs for new and returning patients, including medication history, indications, effectiveness, safety, and adherence.

You'll provide MTM and clinical pharmacy consultation — developing medication-related recommendations, supporting chronic disease medication management, and providing prescriber consultation on medication optimization.

Between consultations, you'll support patient medication counseling, prior authorization, and formulary management, and document clinical pharmacy care accurately in pharmacy documentation systems and EHR.

Throughout the day, you'll coordinate with providers, care teams, and pharmacy teams, and support clinical pharmacy compliance with pharmacist scope, state licensure, and evidence-based pharmacotherapy guidelines.

The role combines comprehensive medication review, medication therapy management, chronic disease medication management, documentation, and medication safety.

Unlike Pharmacy Technician work, which focuses on technician-level pharmacy support, clinical pharmacist work focuses specifically on comprehensive medication review, MTM, and clinical pharmacy consultation — you're the medication expert who helps patients achieve medication outcomes through effective clinical pharmacy care.
