Remote Telehealth Triage RN (Advanced)

Conduct advanced triage assessments, apply complex triage protocols, and lead RN-led emergency escalation — from home.

$35–$60 / hour 100% Remote Full-Time / Part-Time Multiple U.S. States

Job Overview

Virelia Health connects experienced registered nurses with opportunities across advanced telehealth triage, complex clinical triage, and nurse triage leadership.

We are seeking qualified Advanced Telehealth Triage RNs for remote opportunities supporting advanced triage programs, health systems, telehealth companies, health plans, and nurse triage vendors.

A Remote Telehealth Triage RN (Advanced) conducts advanced triage assessments, applies complex triage protocols, and leads RN-led emergency escalation. Working within their state licensure and clinical scope of practice, the advanced triage RN supports patients with complex clinical presentations — applying advanced triage protocols, supporting clinical decision-making, and mentoring less experienced triage nurses.

This position is well-suited to an experienced registered nurse with emergency, critical care, or acute care experience and prior triage experience who wants to practice in a virtual setting — providing advanced clinical triage work with schedule flexibility and without the demands of a facility-based environment.

The specific advanced triage program, patient population, triage protocols, and scope of responsibilities will depend on the employer and assignment.

This opportunity may be suitable for professionals searching for remote advanced triage RN jobs, work-from-home senior triage nurse jobs, remote complex triage RN jobs, advanced telehealth triage RN, or remote senior triage nurse roles.

Advanced Triage Coverage with Predictable Hours

Advanced virtual triage runs on scheduled shift coverage — because triage operates across extended hours. Advanced triage programs typically offer daytime, evening, overnight, or weekend shifts to cover patient demand, with defined shift blocks of 8–12 hours. Some programs offer flexible part-time and PRN options, while others offer full-time schedules. Every advanced triage program provides structured training, supervision, and shift support — advanced triage is done within a team, not alone. We offer both full-time and part-time positions. Discuss your preferred schedule during your interview.

What You'll Do

As a Remote Telehealth Triage RN (Advanced), you may support patients across a range of complex triage encounters.

  • Conduct advanced triage assessments via video or phone — assessing complex symptoms, history, and urgency.
  • Apply advanced nurse triage protocols (Schmitt-Thompson, Barton, or employer-specific) to guide clinical decision-making.
  • Determine the appropriate level of care — self-care, provider visit, urgent care, or emergency services.
  • Lead emergency escalation for complex presentations — directing patients to 911, EMS, or emergency care as clinically indicated.
  • Support complex clinical decision-making — including ambiguous presentations, high-risk populations, and multiple comorbidities.
  • Provide patient education on conditions, symptom management, and self-care.
  • Document each triage encounter accurately in the triage platform or EHR.
  • Mentor and support less experienced triage nurses — providing clinical guidance and consultation within scope.
  • Support triage protocol implementation and quality improvement activities.
  • Coordinate with providers, care teams, and on-call clinical staff as needed.
  • Provide reassurance and clear instructions to patients who are anxious about their symptoms.
  • Support triage quality assurance — reviewing triage encounters for protocol adherence and clinical accuracy.
  • Participate in advanced triage education, clinical review, and quality improvement.
  • Maintain current knowledge of advanced triage protocols, clinical guidelines, and licensure requirements.
  • Protect patient privacy and maintain HIPAA compliance across all virtual triage interactions.

Advanced Telehealth Triage Focus

Advanced telehealth triage is a growing area of clinical practice — driven by increased demand for virtual triage, payer investment in nurse triage lines, and the recognition that advanced triage helps patients with complex presentations get the right care at the right time. Advanced triage RNs extend clinical reach, improve triage accuracy, and support less experienced triage nurses.

Depending on the program, advanced telehealth triage may focus on:

Complex clinical triage
Emergency-level triage
High-risk population triage
Advanced triage protocols
Triage nurse mentorship
Triage quality assurance

The advanced triage RN's role is to provide advanced virtual triage within their licensure and scope of practice — including complex assessment, advanced protocol application, clinical decision-making, emergency escalation, triage nurse mentorship, and documentation. Clinical decisions, medication changes, and diagnostic decisions belong to the treating providers; the advanced triage RN supports the patient's triage encounter, leads complex clinical decisions, and coordinates with the care team.

This structure keeps the role focused on advanced symptom assessment, complex protocol application, level-of-care direction, triage nurse mentorship, and appropriate emergency escalation.

Advanced Clinical Decision-Making & Triage Nurse Mentorship

Effective advanced triage depends on strong clinical decision-making and triage nurse mentorship — the RN's clinical judgment and leadership determine how to support complex triage encounters and develop less experienced triage nurses.

Depending on the program and clinical focus, you may support patients and colleagues by:

  • Applying advanced nurse triage protocols accurately to complex clinical presentations.
  • Supporting clinical decision-making — including ambiguous presentations, high-risk populations, and multiple comorbidities.
  • Leading emergency escalation for complex presentations — directing patients to 911 or emergency care as clinically indicated.
  • Providing clinical guidance and consultation to less experienced triage nurses within scope.
  • Supporting triage quality assurance — reviewing triage encounters for protocol adherence and clinical accuracy.
  • Providing clear patient education on conditions, symptom management, and self-care.
  • Documenting each triage encounter accurately and consistently.
  • Escalating emergency-level concerns to 911, EMS, or the appropriate clinical team immediately.
  • Supporting triage protocol implementation and quality improvement.

Clinical judgment, leadership skills, and clear communication are essential — advanced triage RNs are the clinical leaders who support complex triage and develop the next generation of triage nurses.

Advanced Triage Workflow

A Remote Telehealth Triage RN (Advanced) may follow a structured workflow across each complex triage encounter.

Depending on the employer's protocols, you may:

Answer Contact Identify Patient Assess Complex Symptoms Apply Advanced Protocol Determine Level of Care Educate & Direct Document & Mentor

Advanced Emergency Escalation & Patient Safety

If a patient presents with emergency-level symptoms — including chest pain, stroke symptoms, severe respiratory distress, or another emergency — the advanced triage RN must follow the employer's emergency escalation protocol immediately, including directing the patient to call 911 or seek emergency care. Advanced virtual triage does not replace emergency care.

Required Qualifications

  • Active RN license in good standing in the state(s) required by the employer.
  • Extensive clinical experience in emergency, critical care, or acute care nursing, plus triage or telehealth triage experience.
  • Advanced knowledge of nurse triage protocols (Schmitt-Thompson, Barton, or employer-specific).
  • Strong complex clinical triage, clinical decision-making, and emergency escalation skills.
  • Comfortable with advanced triage platforms, EHR systems, and virtual triage workflows.
  • Experience supporting or mentoring less experienced triage nurses preferred.
  • Strong documentation and time-management skills.
  • Ability to recognize situations requiring emergency or clinical escalation.
  • Ability to work independently in a remote environment.
  • Ability to maintain confidentiality and HIPAA compliance.
  • Reliable internet suitable for real-time video and a private workspace suitable for confidential patient conversations.

A Typical Day

A typical day may begin by logging into the triage system, checking in with your shift lead or supervisor, and reviewing any active case notes, complex cases, or follow-up items.

You'll answer advanced triage calls, chats, or video requests as they come in — conducting complex symptom assessments and applying the appropriate advanced triage protocols to determine each patient's level of care.

Between encounters, you'll document each triage interaction accurately, coordinate with providers and care teams, provide clinical guidance to less experienced triage nurses, and support triage quality assurance activities.

Throughout the day, you'll identify emergency-level concerns and follow the employer's emergency escalation protocol immediately — directing patients to 911 or emergency care as clinically indicated.

The role combines complex symptom assessment, advanced protocol application, level-of-care direction, patient education, triage nurse mentorship, documentation, and appropriate emergency escalation.

Unlike entry-level virtual triage nursing, advanced triage nursing focuses on complex clinical presentations, clinical decision-making, and triage nurse mentorship — you're the clinical leader who supports complex triage and develops the next generation of triage nurses.
